高血压患者臂踝脉搏波速改变及其相关危险因素分析

摘　　要：目的了解上海地区正常人群臂踝脉搏波速(baPWV)参考范围,探讨高血压患者baPWV改变及其相关危险因素。方法选自上海地区健康志愿者104例,观察各年龄段baPWV参考范围;入选住院原发性高血压患者237例,采集心血管危险因素,同步测baPWV并分析其与相关危险因素的关系。结果健康人左右两侧baPWV差异无统计学意义(P=0.67),baPWV随年龄增长而增快(P<0.01),男性baPWV大于女性(P<0.05),校正年龄baPWV与收缩压和舒张压正相关(分别r=0.459,P<0.01和r=0.477,P<0.01)。高血压患者baPWV与心血管危险因素聚集程度正相关(r=0.21,P=0.000),分别与年龄、收缩压、舒张压、血糖、胆固醇、低密度脂蛋白呈正相关(P<0.05)。结论年龄、收缩压、心率及心血管危险因素聚集程度是影响baPWV的主要因素;即使在健康人群,baPWV也与血压正相关。Objective To explore the reference value of brachial-ankle pulse wave velocity（baPWV） in Shanghai healthy population,and to investigate the relationship between changes of baPWV and related risk factors in patients with essential hypertension. Methods One hundred and four healthy volunteers in Shanghai were enrolled,and the reference values of baPWV of each age stages were observed.Besides,the cardiovascular risk factors of 237 hospitalized patients with essential hypertension were obtained,and baPWV was simultaneously detected for the analysis of relationship between baPWV and related risk factors. Results For the healthy subjects,there was no significant difference in bilateral baPWV for（P=0.67）,and baPWV increased with age（P〈0.01）.The baPWV of male was higher than that of female（P〈0.05）.After adjustment for age,baPWV was positively correlated with systolic blood pressure（SBP）（r=0.459,P〈0.01） and diastolic blood pressure（DBP）（r=0.477,P〈0.01）.For those with hypertension,there was positive correlation between baPWV and clusters of cardiovascular risk factors（r=0.21,P=0.000）.baPWV was positively correlated with age,SBP,DBP,blood glucose,cholesterol and low-density lipoprotein（P〈0.05）. Conclusion Age,SBP,heart rate and clusters of cardiovascular risk factors are the main influential factors for baPWV.baPWV is also relevant to blood pressure in healthy people.
