社区高血压患者治疗依从性与靶器官损害的相关性研究

摘　　要：目的：探讨社区高血压患者治疗依从性与靶器官损害的关系及其临床意义。方法：选择250名社区高血压患者，根据近1年来服用降压药情况填写Morisky问卷以评价治疗依从性，并区分为依从性好组（104例）和依从性差组（146例）。比较两组患者的靶器官（心、脑、肾）损害情况，分析Morisky评分和靶器官损害的相关性。结果：高血压患者治疗依从性差组的心、脑、肾等靶器官损害较依从性好组重（P〈0．05）。Morisky评分与高血压患者心、脑、肾等靶器官损害程度呈显著正相关（OR=2．0137～2．8036，P〈0．05）。结论：社区高血压患者治疗依从性与心、脑、肾等靶器官损害密切相关，提高治疗依从性对于防治高血压靶器官损害有着重要的临床意义。Objective： To investigate the correlation between therapeutic compliance of patients with essential hypertension （EH） in community and target organ damage and its clinical significance. Methods： A total of 250 EH patients in community were chosen and given the Morisky questionnaire to evaluate the therapeutic compliance according to hypotensive drugs taken in past 1 year. The 250 EH patients were divided to therapeutic compliance good group （ 104 cases） and compliance bed group （146 cases）. The target organ damage including heart, brain and rein in two group was compared. The relationship between the Morisky score and the severity of target organ damage were analyzed. Results： The severity of target organ damage was more significant in compliance bed group compared with compliance good group （P〈0.05）. There was a significant positive relationship between the Morisky score and the severity of target organ damage （OR=2. 0137～2. 8036, P〈0.05）. Conclusion： The therapeutic compliance of EH patients in community is closely related to the severity of target organ damage, therefore, elevating the therapeutic compliance is of great clinical importance to prevent and therapy of target organ damage.
