摘 要:目的:探讨维生素D3联合地塞米松对哮喘大鼠的治疗作用及其机制.方法:实验分为正常对照组(A组)、哮喘模型组(B组)、地塞米松组(C组)和维生素D3联合地塞米松治疗组(D组),每组Wister大鼠10只.用卵白蛋白(OVA)腹腔注射致敏和雾化吸入激发建立哮喘大鼠模型,采用ELISA方法测定大鼠血清中IgE,IL-4,IL-5含量;收集肺泡灌洗液(BALF)进行细胞计数和分类.结果:①大鼠血清IgE,IL-4,IL-5含量B组分别为(362.84±6.26)×103IU/L,(22.66±1.59)ng/L,和(55.31±2.81)ng/L,A组分别为(36.16±2.61)×103IU/L,(8.37±0.66)ng/L,(13.45±1.70)ng/L,B组和A组相比较,差异具有统计学意义(P<0.05);C组分别为(188.25±5.55)×103IU/L,(6.23±0.90)ng/L,(29.64±1.90)ng/L,D组分别为(114.47±6.00)×103IU/L,(2.06±1.30)ng/L,(7.33±1.33)ng/L,C和D组与B组相比均显著降低(P<0.05);D组与C组相比也显著降低(P<0.05);②大鼠BALF中细胞总数及各种炎症细胞B组较A组增加;C组和D组比B组明显减少(P<0.05),且D组与C组相比亦明显减少,差异具有统计学意义(P<0.05).结论:维生素D3联合地塞米松能够有效地治疗哮喘,其机制可能是通过协同抑哮喘大鼠气道炎性反应和降低哮喘大鼠血清中IgE,IL-4,IL-5水平实现的.AIM : To investigate the therapeutic effect of 1 , 25- (OH) 2-D3 combined with dexamethasone on asthma in rats and its mechanism. METHODS: Forty Wister rats were randomly divided into 4 groups : nomal control group ( A ) , asthma model group(B), group intervened by dexamethasone(C), group intervened by a combination of 1,25-( OH)2-D3 and dexamethasone (D). Group B, C and D were asthma models induced by transperitoneal injection and inhalation of ovalbumin ( OVA ). Peripheral blood was collected. Both total counts and the percentage of different cell types were calculated. The concentrations of IgE, IL4 and IL-5 were determined by ELISA. RESULTS: (1)After the stimulation of OVA, concentrations of IgE, IL4 and IL-5 in peripheral blood differed statistically in each group. The concentrations of IgE, IL4 and IL-5 were(362.84 ±6.26) ×10^3 IU/L, (22.66±1.59) ng/L,(55.31 ±2.81) ng/L in B group, and (36.16±2.61) ×10^3 IU/L,(8.37 ±0.66) ng/L, (13. 45 ± 1.07) ng/L in group A, they were statistically different in this two groups ( P 〈 0. 05 ). They were ( 188.25 ± 5.55 ) ×10^3 IU/L, (6.23 ±0.90) ng/L,(29.64 ± 1.90) ng/L in group C, and statistically different from those in group B ( P 〈 0. 05 ). Compared with gronp B, the concentrations of IgE, IL-4 and IL-5 were greatly decreased to(182.75 ± 6. 00) ×10^3 IU/L, (15. 85 ± 1.25) ng/L,(27.85 ±1.26) ng/Lin group D(P〈0.05)(2) Compared with gronp A, both total counts and the percentage of EOS were increased in BALF in group B, but compared with group B, the total counts and percentage were decreased in group C and group D ( P 〈 0. 05 ). CONCLUSION : The combination of dexamethasone and 1,25- (OH)2-D3 may be more effective on the prevention of inflammation in tunica mucosa bronchiorum. Themechanism by which 1,25-( OH)2-D3 acts synergistically with dexamethasone may be that they can reduce the blood levels of IgE, IL-4 and IL-5.
