三叶钢板置入内固定Neer三,四部分骨折愈合过程中年龄和性别因素的影响

摘　　要：吉林大学第一医院骨科2001-03/2006-04收治Neer分型属于三、四部分的肱骨头粉碎性骨折患者32例,男19例,女13例;年龄19～70岁,其中19～49岁15例,50～70岁17例。所有患者采用三叶草状钢板螺丝钉内固定治疗,治疗后随访8个月,结果发现男女两组间的骨折愈合时间和Constant & Murley功能评分差异不大;与年龄低于50岁的患者相比,年龄超过50岁的患者骨折愈合时间明显延长,Constant & Murley功能评分低(P<0.05)。提示三叶钢板固定治疗肱骨头粉碎性骨折,比较适合于年龄低于50岁的患者,年龄超过50岁的患者术后的骨折愈合时间相对延长,肩关节功能恢复不理想。Thirty-two patients with comminuted proximal humerus fractures （Neer Ⅲ or Ⅳ） were admitted to Department of Orthopedics, First Hospital of Jilin University from March 2001 to April 2006. The patients consisted of 19 males and 13 females, aged 19-70 years, including 15 of 19-49 years and 17 of 50-70 years. All patients underwent treatment of cloverleaf plate-screw fixation. During the follow-up for 8 months, union time and scores from Constant ＆ Murley functional scale were similar between male and female groups. Compared with patients aged 〈 50 years, fracture healing time was significantly longer in patients aged 〉 50 years, and the scores from Constant ＆ Murley functional scale were significantly lower （P 〈 0.05）. The results of the study indicate that cloverleaf plate treatment for comminuted proximal humerus fractures is more suitable for patients 〈 50 years old. The healing time in patients 〉 50 years old is relatively longer with unfavorable functional recovery in the shoulder joint.
