心源性猝死心肌细胞凋亡与c—jun基因蛋白的表达

摘　　要：目的探讨心肌细胞凋亡和c-jun基因蛋白在心源性猝死（SCD）中的表达及其法医学意义。方法50例法医尸检案例，其中冠心病猝死组16例，病毒性心肌炎猝死组14例，扩张型心肌病猝死组10例及对照组（非SCD猝死病例）10例。通过TUNEL法和免疫组织化学法对各组心肌细胞凋亡指数、c-jun基因蛋白表达情况的吸光度进行半定量检测，并分析各组之间的差异。结果冠心病猝死组、病毒性心肌炎猝死组、扩张型心肌病猝死组的凋亡指数均显著高于对照组（P〈0．01）。病毒性心肌炎猝死组和扩张型心肌病猝死组的凋亡指数均高于冠心病猝死组（P〈0．01），前两者之间差异无统计学意义（P〉0．05）。冠心病猝死组、病毒性心肌炎猝死组、扩张型心肌病猝死组的c—jun基因蛋白吸光度值均显著高于对照组（P〈0．01）。3组之间的吸光度值差异无统计学意义（P〉0．05）。结论应用TUNEL法检测心肌细胞凋亡和免疫组织化学法检测心肌细胞内c-jun基因蛋白的表达，可作为法医鉴定SCD的重要指标。Objective To explore the significance of myocardial apoptosis and expression of c-jun in sudden cardiac death（SCD） ,and provide a new method for SCD identification. Methods Fifty autopsy cases were divided into 4 groups： coronary heart disease（CHD） sudden death group 16 cases, viral myocarditis sudden death group 14 cases, dilated cardiomyopathy sudden death group 10 cases and control group （ non SCD） 10 cases. The myocardial apoptosis index was detected by TUNEL method and the expression of myocardial c-jun protein was detected by immunohistochemistry. The differences among 4 groups were analyzed. Results Apoptosis index of CHD sudden death group, viral myocarditis sudden death group and dilated cardiomyopathy sudden death group was significantly higher than that of control group （ P 〈 0. 01 ）. While apoptosis index of viral myocarditis sudden death group and dilated cardiomyopathy sudden death group was significantly higher than that of CHD sudden death group （ P 〈 0.01 ） , but there was no significant difference between two groups （ P 〉 0. 05 ）. The expression of c-jun in CHD sudden death group, viral myocarditis sudden death group and dilated cardiomyopathy sudden death group was significantly higher than that of control group （ P 〈 0.01 ）, but there was no differences among three groups （ P 〉 0.05 ）. Conclusions Cardiomyocyte apoptosis and expression of c-jun can be important markers in SCD identification .
