摘 要:目的:探讨肝移植术中血清透明质酸浓度(HA)的变化与供肝低温保存冷缺血时间的关系。方法:根据低温保存时间的不同,将20例行原位肝移植术的患者分为A组(5—6h)、B组(8—9h)和C组(12h),分别在供肝血流再灌注后5、30、60、120和180min,取3组患者血清检测HA浓度。结果:供肝血流再灌注后,患者血清HA浓度逐步下降。A组平均下降速度为4.58μg/L·min^-1,B组为4.16μg/L·min^-1,C组为2.26μg/L·min^-1。A组和B组HA平均下降速度比较的差异无统计学意义(P〉0.05);与C组相比,A组、B组平均下降速度的差异均有统计学意义(P〈0.05)。结论:供肝低温保存导致的肝内皮细胞损伤程度与供肝低温保存的时间呈正比,并以患者血清HA的下降速度表现出来。Objective :To probe the correlation of intra-operative hyaluronic acid (HA) changes with hepatic allografts cold ischemia reperfusion injury in liver transplantation. Methods:The patients with orthotopic liver transplantation were assigned to 3 groups: (1) group A :liver allografts were stored in University of Wisconsin' s solution (0℃) for 5 -6 h before implantation; (2) group B :liver allografts were stored in U W's solution (0℃) for 8 -9 h before implantation; (3) group C :liver allografts were stored in U W's solution (0℃) for 12 h before implantation. The serum samples were taken up at 5 min ,60 min, 120 min, 180 min after the donor liver was transplanted and reperfused to detect the concentration of HA. Results:After donor liver was transplanted and reperfused ,serum HA concentration decreased immediately and gradually. HA decreased rate :group A 4. 58 μg/L · min^-1, group B 4. 16 μg/L · min^-1, group C 2.26 μg/L · min^-1 Serum HA decrease rate was faster in group A and B than that in group C (P 〈 0.05). Conclusion:The degree that hepatic allografts cold ischemia-reperfusion injure lead to injur of endothelial cell can be indicated by the serum HA concentration decreasing rate after reperfused in liver transplantation.
