新生儿海绵窦外侧壁的应用解剖

摘  要:目的了解新生儿海绵窦外侧壁的解剖结构,为影像学诊断和神经外科学提供解剖学资料。方法对20例(40侧)新生儿海绵窦外侧壁结构在显微镜下进行解剖和测量。结果海绵窦外侧壁为两层结构,外层为硬脑膜,内层为动眼神经、滑车神经和三叉神经分支的神经鞘膜,外侧壁深层不完整者占625%,均在Mullan三角前部或Parksjn三角后部。滑车神经在海绵窦分三型,即交叉型、凸型和直型。结论滑车神经在海绵窦段行程类型不同可影响Parksin三角的显露。选择海绵窦外侧壁入路时Parksin三角最常用,在海绵窦前半部可联合应用Mullan三角。Objective To provide anatomic data for imaging diagnosis and neurosurgery of cavernous sious(CS). Methods The paries laterlis structures of CS in 20 newborns through the microscope. Rosults The lateral wall of CS consisted of two layers: outer layer was cerebral dura mater, and inner layer was lamellar sheath of the oculomotor nerve,trochlear nerve and trigeminal nerve. The stratum profundum ofparies lateralis in CS was incomplete (62.5%), they were in the front of the Mullan triangle or in the back of Parkinson triangle, The trochlear nerve could be divided into three types in the section of CS: cross-over, convex and straight. Conclusion The different types of the trochlear nerve in the section of CS can effect the appearance of the Parkinson triangle.The Parkinson triangle is often used in the operation, and it can combine with Mullan triangle when the operation is in the half front of the CS.
