摘 要:目的探讨2型糖尿病患者中尿基质金属蛋白酶-9(MMP-9)排泄的变化及其临床意义。方法测定88例糖尿病患者和28例正常对照尿MMP-9和尿肌酐(Ucr)比值。2型糖尿病患者据其24h尿白蛋白排泄率(UAER)分为3组:正常蛋白尿组:UAER〈20μg/min,28例;微量蛋白尿组:UAER20~200μg/min,32例;临床蛋白尿组:UAER〉200μg/min,28例。结果与正常对照组比较,糖尿病患者各组尿MMP-9/Ucr明显增高,差异有显著性(P〈0.05)。糖尿病患者正常蛋白尿组尿MMP-9/Ucr比值已升高(P〈0.05),并且随肾脏病情加重进一步增高(P〈0.01)。相关分析显示:糖尿病患者尿MMP-9/Ucr比值与尿白蛋白/Ucr比值(r=0.566,P〈0.01)和尿视黄醇结合蛋白(RBP)/Ucr比值(r=0.692,P〈0.01)呈正相关。结论糖尿病患者尿MMP-9排泄增加与肾脏病变程度有关,尿MMP-9排泄增加可能间接反映糖尿病肾组织MMP-9合成和表达增加。Objective To investigate the changes in urinary matrix metalloproteinase-9 (MMP-9) excretion in patients with type 2 diabetes mellitus. Methods Urinary MMP-9 and creatinine were measured in 88 patients with type 2 diabetes mellitus and 28 control subjects. The diabetic patients were divided into 3 groups according to urinary albumin excretion rate ( UAER ) : ( 1 ) normal albuminuria group, UAER 〈 20 μg/min, n = 28 ; ( 2 ) microalbuminuria group, UAER 20 - 200 μg/min, n = 32 ; (3) macroalbuminuria group, UAER 〉 200 μg/min, n = 28. Results Compared with that in the control group, the ratio of urinary MMP-9/urinary creatininr (Ucr) increased significantly ( P 〈 0.05 ) in diabetes patients. Urinary MMP-9/Ucr had already increased significantly in patients with normal albuminuria (P 〈 0. 05 ) and further rosed as the nephropathy progressed (P 〈 0. 01 ). Urinary MMP-9/UCr was positively related to urinary albumin/Ucr ( r = 0. 566, P 〈 0. 01 ) and urinary retinal binding protein/UCr(r =0. 692,P 〈 0. 01 ). Conclusion Urinary MMP-9 excretion increased in type 2 diabetics, which is related to the degree of diabetic nephropathy; Increased urinary MMP-9 excretion may reflect the increased expression and synthesis of renal tissue of diabetics.
