摘  要:目的:观察rhKD/APP对实验性大鼠急性坏死性胰腺炎(ANP)血清炎性因子和胰腺组织超微结构的影响。方法:采用3.5%的牛磺胆酸钠0.1mL·100g-1逆行注入大鼠胰胆管内诱导ANP模型。60只大鼠随机分为假手术组(Sham组),模型组(Model组),rhKD/APP低、中、高剂量组(4×104kU·kg-1、8×104kU·kg-1、16×104kU·kg-1组),阳性药抑肽酶组(Aproitin组),观察ANP大鼠血清肿瘤坏死因子-α(TNF-α)、一氧化氮(NO)变化和胰腺组织超微结构变化。结果:与假手术组比较,模型组大鼠血清TNF-α和NO水平明显升高(P<0.001);与模型组比较,rhKD/APP中、高剂量组大鼠血清TNF-α和NO均降低(P<0.05或P<0.01),与阳性药抑肽酶组比较差异无显著性。胰腺组织超微结构显示细胞表面可见较多微绒毛,核仁明显,分泌颗粒增多。结论:rhKD/APP早期给药可显著减轻胰腺炎症反应,对大鼠急性胰腺炎具有治疗作用。Objective To observe the effect of rhKD/APP on inflammatory cytokines and uhrastructrue in experimental acute necrosis pancreatitis (ANP) in rats. Methods ANP model was induced by injections of 3.5 % sodium taurocholate into the main pancreaticduct of rats. Sixty rats were randomly divided into 6 groups (n=10) : sham group; model group; low, mediate, high dose rhKD/APP groups (4 × 10^4, 8 × 104 and 16 × 10^4 kU·kg 1);aproitin group. TNF-α and NO activity were assayed and ultrastructrue changes were observed after treatment with rhKD/APP. Results Compared with sham group, the levels of TNF-α and NO in serum in model group rapdily raised (P〈0.01). Compared with model group, the levels of serum TNF-α andNO in mediate and high dose rhKD/APP groups were decreased (P〈0.05 or P〈0.01); but there was no significant difference compared with aproitin group. The uhrastructure results of pancreas tissue showed that there were a lot of microvilli on cell surface, the nucleoli were significant, and secretory granules inreased. Conclusion rhKD/ APP can reduce the inflammatory response of pancreas, and has therapeutic effect on rats with ANP.
