卡维地络对高龄慢性心力衰竭凋亡基因PDCD5及sFas表达的影响

摘　　要：目的观察卡维地络对高龄(>75岁)慢性充血性心力衰竭(CHF)患者凋亡基因PDCD5及sFas表达的影响。方法选择我院2005年6月～2007年10月108例住院患者,随机分为2组,常规治疗基础上分别给予卡维地络递增剂量治疗(治疗组)和安慰剂治疗(安慰剂组),另设正常对照组50例(正常组)。观察卡维地络治疗高龄CHF患者的疗效,外周静脉血凋亡基因PDCD5血清抗体变化及血清可溶性Fas(sFas)的变化。结果慢性心力衰竭患者血清PDCD5抗体、血清可溶性Fas水平高于正常组,治疗后治疗组血压、心率下降,左室射血分数(LVEF)增加,PDCD5抗体滴度、sFas水平降低。结论卡维地络适于高龄心力衰竭患者可改善心功能,卡维地洛治疗后基因PDCD5及sFas下降提示卡维地洛具有抗细胞凋亡作用。Objective To observe the influence of carvedilol on the expression of apoptosis gene PDCD5 and sFas in senile patients with chronic heart failure （CHF）. Methods 108 senile patients with CHF were randomly divided into two groups, including placebo group and carvedilol group treated with routine therapy alone or additional carvedilol, respectively. 50 health people were as normal control. The changes of serum PDCD5 antibody in patients with CHF and the effect of carvedilol were observed. Results The levels of PDCD5 and sFas antibody in patients with CHF were significantly higher than that in health people. After therapy of carvedilol, BP and HR decreased （P〈0.01）, and LVEF increased （P〈0. 05）. The apoptosis gene PDCD5 and sFas antibody decreased. Conclusion The results show that carvedilol may improve the heart function in the senile patients with CHF and influence the expression of some apoptosis genes.
