高度流行区农村外出务工人员血吸虫病病情调查及防制对策研究

摘　　要：目的了解和掌握湖沼垸内型高度流行区农村外出务工人员血吸虫病病情,为制定和采取有针对性防制策略和措施提供科学依据。方法对潜江市张金镇农村外出务工人员进行问卷调查和IHA、COPT及Dot-ELISA检测,分析不同性别、年龄、外出地区和外出期限等人群血吸虫感染情况,同时开展血防健康教育、查治病和建立血防健康档案等。结果该镇农村外出务工人员占农村总人数的33.39%,20岁以上为主要群体,血检阳性率为20.94%,男性高于女性,分别为23.30%和18.99%。在本地市镇企业务工者和在广东、上海及江浙等沿海地区务工者的阳性率分别为24.31%和17.79%;30岁以上年龄组高于其他年龄段,小学以下和高中以上文化程度的感染率分别为23.78%和19.11%。结论由于漏查漏治,湖沼垸内型疫区农村外出务工人员的血吸虫病病情是严重的,疫区政府和血防主管部门要加强对该类人群的管理和查治力度,杜绝和控制其传染源向非疫区城市扩散和传播。Objective To understand the epidemic situation of the rural migrant workers from heavily schistosomiasis endemic marshland areas,in order to provide data for schistosomiasis control. Methods Questionnaire and serological test were carried out among the rural emigrant workers, and the infection situation was analyzed according to gender,age, the migrant areas and migrant time period,at the same time,health education,ease screening and treatment were adopted, and health archive was established. Results The migrant workers account for 33.39% of the population in the endemic region, of which most were above 20 years old, the total positive rate of the serological test was 20. 94%, and that of male and female was 23. 30% and 18. 99% respectively. Positive rate of people working in the local enterprises and working in costal areas （ such as Guangdong 、Shanghai Jiangsu and Zhejiang etc） was 24. 31% and 17.79% respectively; the rate of workers above 30 years old was higher than that of other ages ; the infection rate of the migrant workers being educated under primary school and above senior middle school was 23.78% and 19. 11% respectively. Conclusion As a result of being missed in screening and treament in this particular population, the infection situation of the migrant workers was quite serious, the local government and its department of schistosomiasis control should pay higher attention to it, and should give enough eoneeru to the migrant workers as well,so as to put the disease under control and avoid spreading to other non-endemic areas.
