摘 要:目的探讨原发性高血压患者高脂肪餐后各时间点血清TG水平及其增长值与餐后TG曲线下面积(TG-AUC)的关系,分析体重指数(BMI)与代谢综合征(MS)相关指标的关系。方法26例高血压病患者禁食12 h后进行高脂肪餐负荷试验,采集空腹和餐后2、4、5、7 h静脉血检测血脂,并测定空腹和餐后2 h血清胰岛素(Ins)、C肽及空腹血尿酸(UA)。结果餐后2、4、5、7 h TG水平明显升高,TC、HDL-C无明显变化;餐后2、4、5、7 h TG增加值与TG-AUC均呈正相关,其中4、5 h TG增加值(△TG4h、△TG5h)与TG-AUC的相关系数最大;高血压患者BMI与UA、空腹TG、餐后2h TG、Ins、餐后2h Ins、C肽、餐后2h C肽及稳态模型胰岛素抵抗指数(HOMA-IR)均呈显著正相关。结论原发性高血压患者△TG4h、△TG5h均可作为TG-AUC的替代指标;BMI与尿酸代谢异常、TG代谢异常、高胰岛素血症、胰岛素抵抗状态这些MS组分密切相关。Objective To analyze some points of TG level and the increase value related to TG-AUC on patients with primary hypertension after a high-fat meal, as well as the correlation between BMI and the components of metabolic syndrome. Methods Twenty-six patients with primary hypertension received a standardized fat loading test after a 12-hour overnight fast, venous blood samples were respectively collected at 0, 2, 4, 5and 7h after fat loading for blood fat, including fasting and 2h postprandial insulin, C-peptide and fasting blood uric acid. Results The level of TG at 2, 4, 5 and 7h after fat loading increased obviously, while postprandial TC and HDL had no significant change. The increase value of TG at 2, 4, 5 and 7h after loading all showed positive correlation with TG-AUC, especially strongest in the increase value at 4 and 5 h (△TG4h, △TG5h). BMI was significantly positive-correlated with UA, fasting and 2h postprandial TG, insulin amd C peptide, as well as the homeostatic model assessment of insulin resistance (HOMA-IR) on patients with primary hypertension. Conclusion Both △TG5h and △TG4h of patients with primary hypertension can stand for TG-AUC as simple parameters. BMI is closely correlated with the components of metabolic syndrome such as abnormality of uric acid, dyslipidemia, hyperinsulinemia and insulin resistance.
