摘  要:目的探讨阻塞性睡眠呼吸暂停低通气综合征(OSAHS)与胰岛素抵抗的关系。方法随机选择OSAHS患者48例,健康人36例,分为OSAHS肥胖组患者28例,对照16例;OSAHS非肥胖组患者20例,对照20例。所有研究对象均进行整夜睡眠呼吸监测(>7小时),空腹血糖(FPG)及空腹胰岛素(FINS)测定,并记录睡眠呼吸暂停低通气指数(AHI)、血氧饱和度等。结果无论是肥胖组或非肥胖组,OSAHS患者的FPG、FINS水平均明显高于对照组;OSAHS患者的胰岛素抵抗指数(HOMA-IR)高于对照组,差异有统计学意义;OSAHS患者的AHI与FINS及HOMA-IR呈正相关。结论OSAHS独立于肥胖等混淆因素,与胰岛素抵抗存在相关性。Objective To explore the relationship among obstructive sleep apnea-hypopnea syndrome (OSAHS) and insulin resistance. Medthods Forty-eight patients with OSAHS were selected as trial group, and 36 normal subjects as control group. The trial group were devided into two groups according to they were obesity or not. The control group were devided into two groups according to they were obesity or not. Polysomnography was examined during sleep, and fasting plasma glucose (FPG) and fasting plasma insulin (FINS) were detected in two groups. Results Fasting plasma glucose, fasting plasma insulin and HOMA-IR were significantly higher in OSAHS group than in control group (P〈0. 05) regardless of they were obesity or not. HOMA-IR and FINS were significantly positively correlated to apnea hypopnea index (AHI). Conclusion OSAHS is independent of obesity and is strongly associated with insulin resistance.
