老年2型糖尿病患者下肢静脉瓣功能不全研究

摘　　要：目的:研究糖尿病对老年糖尿病患者下肢静脉瓣膜功能损伤。方法:133例老年糖尿病患者行临床、双下肢静脉彩色多普勤检查,部份同时行下肢静脉顺行造影,结果用 CEAP 评分,与对照组比较。结果:老年 DM 组 CEAP 各项评分中临床表现严重程度及功能丧失评分明显高于对照组。老年糖尿病组受累血管以小腿部腘、胫前、胫后静脉,大隐静脉膝下段、小腿部交通静脉返流性受累多于对照组(P<0.01),静脉瓣返流组合也以小腿部静脉明显。结论:老年糖尿病患者下肢慢性静脉瓣功能不全重于一般老年人,受累静脉损伤以小腿部静脉为主。提示糖尿病患者下肢静脉损伤为糖尿病足发病原因之一,应早期采取措施进行防治。Objective： To investigate the impaired function of veins valve in lower limb in elderly patients with type 2 diabetes mellitus. Methods： One hundred and thirty-three elderly patients with type 2 diabetes mellitus were detected by doppler ultrasonic of veins of lower limb. The result of compared with control group through the CEAP score. Results： The mark of serious degree of clinical symptom and function were significantly higher than control group in CEAP score of elderly patients with diabetes. In the group of elderly patients with diabetes, back flow of venous valve in involved veins were more titan control group （ P 〈0.01 ）. The involved veins mainly include popliteal vein, anterior tibial vein, posterior tibial vein, great saphenous vein and traffic vein in calf. Conclusion： The injury of function of old diabetes patients＇ veins valve of lower limb were more serious than normal old people. Veins of leg were mainly involved. The injury of vein of lower limb on diabetes patients was one of reasons of diabetic foot, to whom active prevention and cure should be given.
