摘 要:目的:与组织学对照,探讨自体骨软骨移植术后受区的高分辨率MR表现特点。材料和方法:依术后随访时间将10只健康成年山羊随机分为5组。在股骨髁负重面制造一直径为8mm的圆柱形骨软骨缺损,自非负重面取3个直径为3mm的骨软骨柱并以马赛克方式填入缺损区。采用Philips Achieva3.0T磁共振扫描仪进行高分辨率MR成像。结果:受区完全修复的最早时间点为术后8周。所有移植物(除术后12周3例外)均存活。各个时间点上,移植物与邻近骨软骨之间的软骨-软骨匹配率均显著高于骨-骨。修复组织早期表现为高信号,随时间进展逐渐变为低信号。组织学显示移植物基本保持原有结构,修复组织为纤维软骨和(或)纤维组织。结论:自体骨软骨移植可修复骨软骨缺损,高分辨率MRI可无创性动态观察软骨修复过程。Purpose: To investigate the high- resolution MR characterization of recipient site after autologous osteochondral transplantation (AOT) compared with histology.Materials and Methods: 10 healthy, mature goats were randomly divided into five groups according to the follow - up time after surgery, A cylindrical osteochondral defect (Ф8mm) was surgically created on the weight - bearing surface of femoral condyle, three osteochondral grafts (Ф3mm) from the non - weight - bearing surface were transplanted to the defect with a mosaic fashion, High - resolution MR follow - up examinations of stifles were initiated on a 3.0T MR unit(Philips Achieva) . Results: The earliest time point of complete fill on recipient site was 8 weeks after surgery. All the grafts (except 3 grafts at 12- week ) kept intact, The rate of cartilage - cartilage match was statistically higher than bone - bone between grafts and native osteochondral tissue at each time point. The repair tissue at early time points showed hyperintense and gradually decreased to hypointense. Histologically, the grafts were basically intact and the defect predominantly comprised fibrocartilage and/or fibrous tissue. Conclusion: AOT can completely fill the osteochondral defect. High- resolution MR imaging provides a non - invasive tool for monitoring the process of repair after AOT.
