子宫全切除术对血清抑制素B水平的影响

摘　　要：目的探讨子宫全切除术后抑制素B（INHB）、雌激素（E2）、卵泡刺激素（FSH）的变化。方法因子宫肌瘤行保留双侧卵巢的子宫全切除患者30例（试验组），于术前月经第5-7天及术后6个月同日晨空腹抽取静脉血，测定FSH、E2及INHB。另选择同期在门诊进行体格检查的健康妇女30名，于月经第5-7天测定FSH、E2及INHB，作为正常参考值。结果术前，试验组E2水平显著高于正常参考值（P〈0.05），而FSH、INHB水平的差异无统计学意义（P值均〉0.05）。术后，试验组的FSH水平较术前显著增高（P〈0.05），E2、INHB显著下降（P值均〈0.01）；E2、FSH水平与正常参考值的差异无统计学意义（P值均〉0．05），INHB水平显著低于正常参考值（P〈0.01）。INHB的下降幅度为（47．4±24．2）％，显著高于E2的（18．2±52．5）％（P〈0.05）。结论子宫全切除术后半年卵巢功能衰退，且术后血清INHs的下降幅度较E2更明显，INHB可能成为反映卵巢功能衰退更显著、更敏感的指标。Objective To observe the changes of serum inhibin B（INHB）, estrogen （E2）, and follicle-stimulating hormone （FSH） levels in patients with hysteromyoma after the uterectomy. Methods Thirty patients,whose uteruses were excised due to hysteromyoma, served as experimental group. The fasting blood samples of all the patients were collected on cycle day 5-7 before operation and the same cycle days 6 months after operation to determine the levels of FSH, E2, and INHB. Thirty healthy women, who underwent physical examination at same period, served as controls. Results The E2 level was significantly higher in the experimental group than in the normal control group before operation （P〈0.05） ; the serum levels of FSH and INHB were similar between the 2 groups. Compared with those before operation,the FSH level was significantly increased 6 months after uterectomy in the experimental group （P〈0.05）; the F4 and INHB levels were significantly decreased （P〈0.01）. The levels of E2 and FSH were not significantly different from those of the normal controls,and the level of INHB was significantly lower than that of the normal control （P〈0.01）. The INHB decreased by （47.4±24.2）%,significantly higher than that of the E2 （18.2±52.5）% （P〈0.05）. Conclusion The ovary function declines half a year after uterectomy. The decrease of serum INHB is greater than that of serum E2 after operation,which might be a more effective and sensitive marker for decline of ovary function.
