摘 要:目的探讨骨髓间充质干细胞(bone mesenchymal stem cell,BMSC)复合藻酸盐凝胶支架修复兔退变椎间盘的效果。方法建立兔腰椎间盘退变模型,并将其分为对照组、空白组和治疗组。体外培养兔骨髓间充质干细胞,治疗组于椎间隙注射BMSC复合藻酸盐凝胶支架,对照组注射藻酸盐凝胶,空白组不予处理。应用MRI、免疫组织化学和生化分析的方法,观察椎间盘退变修复效果。结果术后2、4、8和12周,3组腰椎间盘均发生不同程度退变,术后12周治疗组的退变程度明显轻于对照组和空白组(P值均<0.05)。12周后,空白组、对照组及治疗组椎间盘髓核蛋白多糖的含量分别为(4.982±0.458)、(5.318±0.602)和(6.487±0.609)mg/100 mg组织,治疗组显著高于空白组和对照组(P值均<0.05);空白组、对照组及治疗组的椎间盘细胞外间质灰度值分别为150.669±2.387、149.849±2.481和141.680±1.618,治疗组Ⅱ型胶原含量显著高于对照组和空白组(P值均<0.05)。结论BMSC复合藻酸盐凝胶支架具有修复腰椎间盘退变的能力,可以做为修复椎间盘退变的生物学方法。Objective To evaluate the outcomes of bone mesenchymal stem cells (BMSCs) entrapped in alginate bead in repairing the degenerative disc. Methods The rabbit models of degenerative intervertebral disc were established by nucleus aspiration and were divided into control group, blank group and treatment groups. Encapsulated in alginate beads, BMSCs were transplanted into the degenerative discs in the treatment group; animals in the control group only received alginate beads, and those in the blank group received no treatment. The repairing effects on the degenerative discs were determined by MRI, immunohistochemical and biochemical analysis in the 3 groups. Results Degeneration of lumbar discs occurred in 3 groups 2, 4, 8 and 12 weeks after operation; while 12 weeks after operation, the degeneration in the treatment group was significantly slighter than those in the control group and blank group (both P〈0.05). Twelve weeks later, the contents of proteoglycan were (4.982±0.458), (5. 318±0.602) and (6.487±0.609) mg/100 mg in the blank group, control group and treatment group, respectively. The proteoglycan levels in the treatment group were significantly higher than those in the control group and blank group (both P〈0. 05). The contents of type Ⅱ collagen were 150.669±2.387, 149.849±2.48 and 141.680±1.618 in the blank group, control group and treatment group, respectively. The type Ⅱ collagen level in the treatment group were significantly higher than that in the control group and blank group (both P〈0.05). Conclusion Alginate scaffold combined with BMSCs may be useful for repairing the degeneration disc, which may serve a biological method for repair of degraded discs.
