腹腔镜手术治疗输卵管妊娠78例临床分析

摘　　要：目的探讨腹腔镜手术在治疗输卵管妊娠中的临床应用价值。方法收集杭州师范大学附属医院2005年1月至2007年6月期间腹腔镜治疗输卵管妊娠病例78例,随机选择同期本院开腹手术治疗输卵管妊娠44例为对照组。记录其术前病史、阳性体征、手术时间、术中出血量、术后抗生素使用时间、术后肛门恢复排气时间、住院费用和住院天数。手术方式:输卵管妊娠部位切开取胚术、输卵管妊娠破裂清理病灶修补术及患侧输卵管切除术。结果腹腔镜手术组术中出血量少(t=8.993,P<0.05)、手术时间短(t=8.533,P<0.05)、术后肛门恢复排气时间短(t=11.979,P<0.05)、术后抗生素使用时间短(t=9.548,P<0.05)、术后住院时间短(t=7.744,P<0.05),但手术费用较高(t=6.580,P<0.05)。结论腹腔镜手术是治疗输卵管异位妊娠的一种理想的手术方式,值得推广。Objective To evaluate the application of laparoscopic surgery in the treatment of tubal ectopic pregnancy. Methods 122 patients diagnosed of tubal ectopic pregnancy were divided into television laparoscopic surgery group （78 cases） and laparotomy group （control group, 44 cases）. The preoperative history, positive signs, operative time, blood loss during operation, duration of postoperative antibiotics applying, recovery time of anal exsufflation, cost of hospitalization and time of hospitalization were recorded and analyzed. Operations included taking embryos from tubal pregnancy site, clearing and repairing tubal pregnancy rupture,and the affected tubal resection. Results Blood loss during operation was less in the television laparoscopic surgery group than that in the control group （ t = 8. 993, P 〈 0.05 ）. Operative time was less in the television laparoscopic surgery group than that in the control group（ t = 8. 533, P 〈 0.05 ）.The recovery time of anal exsumation was shorter in the television laparoscopic surgery group than that in the control group （t = 11. 979, P 〈 0.05 ）. Duration of postoperative antibiotics applying was shorter in the television laparoscopic surgery group than that in the control group（ t = 9. 548,P 〈 0.05 ）. Time of hospitalization was shorter in the television laparoscopic surgery group than that in the control group（t =7. 744,P 〈 0.05） , however, the cost of hospitalization was higher in the television laparoscopic surgery group than that in the control group （ t = 6. 580, P 〈 0.05 ）. Conclusion Laparoseopic surgery is an ideal operation for the treatment of tubal ectopic pregnancy and it is worthy of promotion.
