蹲式体位对正常骨盆形态产程活跃期胎头浮动产妇分娩的影响

摘　　要：目的：探讨对正常骨盆形态产程活跃期胎头浮动产妇实施蹲式体位对分娩效果的影响。方法：选择产程活跃期宫口开大〉3cm经阴道检查确诊胎头浮动产妇200例，随机分为观察组和对照组各100例，观察组当宫口开大3—4cm时，在宫缩期间嘱产妇取蹲式体位，并由助产士给予指导；对照组取平卧位或侧卧位直至宫口开全，不予体位指导至分娩结束。结果：观察组第一、二产程时间、顺产率及剖宫产率与对照组比较有极显著性差异（P〈0．01），两组产后出血、新生儿窒息率比较有显著性差异（P〈0．05）。结论：对正常骨盆形态产程活跃期胎头浮动产妇实施蹲式体位，可缩短产程，促进自然分娩，降低剖宫产率，减少母儿并发症，值得临床推广应用。Objective ： To explore the effect of squatting posture on pregnant women with normal pelvic and having fetal head floating in active period of labor stages. Methods： Two hundred pregnant women whose ostium of uterus opened more than 3 cm and had fetal head floating by trans - vaginal diagnosis in active period of labor stages were randomly divided into observation group and control group （ 100 for each group）. The pregnant women in observation group were asked to take squatting posture when their ostium of uterus opened in 3 - 4 cm and midwives gave them instruction on delivery, the pregnant women in control group took horizontal or lateral recumbent position till their ostium of uterus opened thoroughly and gave birth to a child, without any instruction from midwives. Results： There were significant differences in the comparison of the time of the first and second labor stage, the rate of normal labor and cesarean section between observation group and control group （ P 〈 0. 01 ） ; the significant differences were also found in the comparison of flooding and the incidence of neo- natal asphyxia between the two groups （ P 〈 0. 05 ）. Conclusion ： It can shorten labor stage, accelerate natural labor, reduce the rate of cesarean section and decrease maternal and neonatal complications for pregnant women with normal pelvic and having fetal head floating in active period of labor stages to take squatting posture for delivery.
