曲马多联合芬太尼用于妇科子宫全切术后镇痛的临床观察

摘　　要：目的:观察曲马多联合芬太尼用于妇科子宫全切术后镇痛效果。方法:将180例子宫全切术患者随机双盲分为曲马多组(A组)、芬太尼组(B组)和芬太尼+曲多组(C组)。术后分别以相应药物静脉泵持续给药,分别观察术后2,5,8,11,24,48 h视觉模拟评分(VAS),不同时间段PCA按压次数、镇痛并发症、总体满意度及肠蠕动恢复时间。结果:曲多组(A组)、芬太尼组(B组)两组与芬太尼+曲马多组(C组)在术后VAS疼痛评分、不同时间段PCA按压次数、镇痛并发症、总体满意度及肠蠕动恢复时间方面均有显著差异(P<0.05或P<0.01),曲马多组(A组)与芬太尼组(B组)第24 h VAS疼痛评分比较有显著差异(P<0.05),余均无显著差异(P>0.05)。结论:曲马多联合芬太尼用于妇科子宫全切术后镇痛效果满意、并发症少、患者总体满意度高,值得临床推广应用。Objective： To observe the analgesic effect of tramadol combined with fentanyl in patients after total hysterectomy. Methods： Patients with total hysterectomy （n = 180） were randomly divided 3 groups, and treated with tramadol, fentanyl and tramadol ＋ fentanyl, respectively. The drugs were continuously administered via a vein pump after the operation. At 2, 5, 8, 11, 24 and 48 h after the operation, VAS pain grade, PCA pressing time, complications of analgesics, the overall degree of satisfactory, and recovering time for enterokinesia were observed. Results： There were significant differences among 3 groups （P 〈0.05 or 0.01 ） in PCA pressing time, complications of analgesics, overall degree of satisfactory and recovering time for enterokinesia. An obvious difference was found in PCA pressing time between patients with tramadol and fentanyl alone 24 h after operation （P 〈 0.05） , and no difference was found for other indicators （P 〉 0.05 ）. Conclusion ： The combination of tramadol and fentanyl has a satisfactory analgesic effect, few complications and a high degree of overall satsfactory in patients after total hysterectomy.
