盆腔脓肿通过腹腔镜诊治的临床处理

摘　　要：目的探讨腹腔镜手术在盆腔脓肿诊治中的价值。方法在腹腔镜下对40例盆腔脓肿行盆腔粘连松解术、输卵管积脓引流或切除,病灶清除、盆腔冲洗等操作,术后应用抗生素治疗。结果40例盆腔脓肿术后症状、体征迅速消失,均痊愈出院,无一例并发症发生,所有病例随访6个月,无一例复发,目前已有7例受孕,3例足月分娩。结论腹腔镜手术结合使用抗生素是目前治疗盆腔脓肿的高效、安全、治愈较彻底的方法,有利于保护患者生育功能,对于未婚患者尤为适用。Objective To investigate the value of laparoscopic operations for the diagnosis and treat- ments of pelvis abscess. Methods 40 patients with hospitalizing diagnosis of pelvic abscess accepted laparo-scopic exploratory operation to isolate pelvic adhesion, resect focus of infection and irrigate in those cases diag-nosed as pelvic abscess during operation. After operation they were treated by antibiotics. Results All patients showed re-mission of pelvic inflammation and physical sign. No one relapse occurred, follow-up checkuping for six months in all patients. 7 patients have gestated and 3 patients have had term deliveries until now. Conclusion Laparoscopy in treating pelvic abscess is feasible for its safety, minimal trauma and quick recovery. Early lap-aroscopy operation is benefit for protection of procreative function, especially for nonparous patients.
