摘 要:目的 测定老年臂丛神经阻滞患者靶控输注舒芬太尼引起呼吸抑制的半数血浆靶浓度(EC50)。方法 随机选择择期行骨科上肢手术老年患者20例,ASA Ⅰ或Ⅱ级,年龄65—75岁,按序贯法靶控输注舒芬太尼30min,初始血浆靶浓度设为0.1“g·L^-1,相邻血浆靶浓度之间比值为1.2。根据呼吸频率、呼吸暂停时间、血氧饱和度(SpO2)、呼气末二氧化碳分压及动脉血气分析来判断呼吸抑制,计算舒芬太尼的EC50及95%可信区间。结果 靶控输注舒芬太尼引起呼吸抑制的EG50为0.20μg·L^-1,95%可信区间为0.18~0.22μg·L^-1。结论 老年臂丛神经阻滞患者靶控输注舒芬太尼引起呼吸抑制的EC50为0.20μg·L^-1。Objective To determine the half-effective plasma concentration (EC50) of sufentanil to cause respiratory depression during brachial plexus block anesthesia for elderly patients. Methods Twenty patients aged 65-75 years, with grade Ⅰ or Ⅱ ASA, undergoing elective operations were enrolled. TCI sufentanil was given by "up and down" method for 30 minutes, started form target plasma concentration of 0. 1 μg· L^-1. And the ratio of two close plasma concentrations was 1.2, RR, SpO2 , PETCO2 and arterial gas analysis were monitored before and during operation. Results The ECho of sufentanil was 0.20 μg· L^-1 The 95% confidence level was 0.18 - 0.22 μg·L^-1 Conclusion The half-effective plasma concentration of suifentanil to cause respiratory depression was 0.20 μg·L^-1.
