摘 要:目的探讨抗生素联合中药治疗腹腔感染时血浆炎症因子水平的变化,为治疗腹腔感染提供新策略。方法选取雄性SD大鼠100只,随机分为假手术组、模型组、泰能组(泰能20mg/kg)、血必净组(血必净8ml/kg)、联合用药组(泰能20mg/kg+血必净8ml/kg),每组20只,采用盲肠结扎穿孔诱发腹腔感染模型。从尾静脉注入不同药物,给药后2、6h从股静脉抽取静脉血2.5ml,离心后测定血清TNF-α及IL-6的变化。结果泰能组血浆TNF-α2h(13.00±1.41)ng/ml较模型组(16.70±2.31)ng/ml低(t=3.153,P<0.01),6h(14.20±2.15)ng/ml与模型组(13.50±2.17)ng/ml差异无统计学意义(t=0.860,P>0.05);血必净组TNF-α2h为(10.30±2.31)ng/ml、6h为(11.40±1.65)ng/ml和联合用药组2h为(10.50±1.43)ng/ml、6h为(10.50±1.08)ng/ml,血必净组、联合用药组2、6h时TNF-α均明显低于模型组(均P<0.01)。模型组血浆IL-6水平2h为(1441.50±190.95)ng/ml、6h为(1305.60±208.04)ng/ml,均高于假手术组(715.40±145.84)ng/ml、(583.60±140.19)ng/m(l均P<0.01),血必净组、泰能组及联合用药组2、6hIL-6均较模型组低(均P<0.01),而联合用药组又较血必净、泰能组低(均P<0.01)。结论联合应用中药血必净和西药泰能治疗腹腔感染,能更有效地降低血浆TNF-α、IL-6水平,是治疗腹腔感染的新策略。Objective To investigate the changes of plasma TNF-α and IL-6 levels in rats with intra-abdominal infection under treatment of antibiotics combined with traditional Chinese medicine. Methods One hundred male SD rats were randomly divided into 5 groups: sham group, control group, imipenem group ( 20mg/kg ), Chinese medicine Xuebijing group( 8ml/kg ), combination group( imipenem 20mg/kg + Xuebijing 8ml/kg )with 20 in each group. Intra-abdominal infection was induced by tigating and puncturing cecum of animals. Blood samples were taken 2 h or 6 h after operation and plasma TNF-α and IL-6 levels were measured. Results Plasma TNF-α levels in control group reached the peak 2 h ( 16.70 ± 2.31 ng/ml) after operation and declined in 6 h( 13.50 ± 2.17mg/ml). In antibiotics group TNF-a levels( 2h : 13.00 ± 1.41 ng/ml; 6h. 14.20±2.15ng/ml) were lower than those in control group (t=3.153, P〈0.01) at 2 h after operation and had no difference at 6 h (t=0.860, P 〉 0.05). In Xuebijing group (2h:10.30 ± 2.31ng/ml; 6h: 11.40 ± 1.65ng/ml) and combination group (2h: 10.50 ± 1.43ng/ml; 6h: 10.50 ± 1.08ng/ml) TNF-a levels were significantly lower than those in control group and antibiotics group (P〈0.01). Plasma IL-6 levels in control group( 2h:1441.50 ± 190.95ng/ml; 6h:1305.60 ± 208.04ng/ml) were significantly higher than those in sham group (2h: 715.40 ± 145.84ng/ml; 6h: 583.60 ± 140.19ng/mi) (P〈0.01). Plasma IL-6 levels in all other groups were lower than control group, and those in combination group was lower than those in antibiotics or Xuebijing group (P〈0.01). Conclusion The combination of Xuebijing and antibiotics (Imipenem) in treatment of intra-abdominal infection can decrease the plasma levels of TNF-α and IL-6 in rats.
