阶段性护理干预对乳腺癌术后患侧上肢功能恢复的影响

摘　　要：目的探讨乳腺癌根治术后阶段性护理指导功能锻炼对患侧上肢功能恢复的影响。方法将100例乳腺癌根治术后患者随机分为对照组和实验组(各50例)。对照组行常规乳腺癌根治术后护理,实验组除常规护理外在术后不同时间点(术后7 d、15 d、30 d、60 d、90 d、180 d)进行徒手肌力综合测定、上肢肿胀测量、患侧上肢运动耐力测试、负压引流情况评估,针对患者具体情况指导其进行功能锻炼。结果实验组的引流总量及引流管拔管天数较对照组增加(P<0.05),徒手肌力综合测定、上肢肿胀测量、患侧上肢运动耐力测试、疼痛情况等指标的统计分析显示实验组优于对照组,差异有显著意义(P<0.05)。结论采用阶段性护理干预对乳腺癌术后伤口的近期恢复(引流总量及引流管拔管天数)虽然有一定的负面影响,但对患侧上肢功能恢复的远期效果例如术后患肢疼痛、徒手肌力、患侧上肢肿胀及上肢运动耐力等有十分重要的作用,能明显改善患者生活质量。Objective To study the effect of periodical nursing intervention on upper limbs function recovery after breast cancer operation, Method 100 patients after breast cancer operation were divided into two groups （50 cases per group）, The patients in control group received regular nursing; The patients in studying group received periodical nursing intervention on upper limbs function recover except regular nursing, then measuring the muscle power, diameter of arm, endurance of exercise, drainage situation （7 days, 15 days, 30 days, 60 days, 90 days and 180 days after operation）, these indicator in both group were statistically analyzed. Result For the studying group, the vol.ume and total, days of drainage increased, but the muscle power, edema of arm, endurance of exercise and pain feeling were better than of in the control group（P〈0.05）. Conclusion Periodical nursing intervention on upper limbs function recovery after breast cancer operation may cause some negative effects on the volume and total days of drainage, but it leads to many positive effects on the muscle power, edema decreasing of arm, endurance of exercise and pain releasing, which is very important for improving life quality after operation.
