摘 要:目的观察乌体林斯(草分支杆菌注射液)预防糖皮质激素(激素)敏感型肾病综合征(SSNS)复发的疗效并探讨其机制。方法选择52例SSNS为研究对象,随机分为观察组(27例)和对照组(25例)。两组均采用国内通用的标准中长程激素方案治疗,观察组在尿蛋白转阴后加用乌体林斯肌肉注射每周1次,共12周。观察两组治疗过程及疗程结束1年内复发情况,两组频复发例数、不频复发例数及无复发例数的差异。另外,观察组15例和对照组13病儿进行了辅助性T淋巴细胞亚群(Th1、Th2)动态检测。结果观察组在维持治疗过程中、疗程结束后1年内复发平均人次均显著低于对照组(t=3.98、3.63,P<0.01),频复发例数和非频复发例数均明显少于对照组(χ2=4.11、6.00,P<0.05),而无复发例数则明显高于对照组(2χ=7.66,P<0.01)。两组病儿激素治疗前Th1、Th2细胞百分率和Th1/Th2比值均无显著性差异(P均>0.05)。观察组经乌体林斯治疗后Th1细胞百分率和Th1/Th2比值均明显高于对照组,差异有显著性(t=2.81、2.16,P<0.05),而Th2细胞百分率虽低于对照组,但差异无显著性(P>0.05)。结论乌体林斯预防SSNS复发疗效确切,其机制与调节Th1/Th2功能平衡,促进SSNS免疫异常的恢复和提高机体抗感染能力等有关。Objective To observe the effects of Wutilinsi injection (Mycohacterium phlei injection) on anti-relapse in steroid sensitive nephrotic syndrome (SSNS) and investigate its mechanism. Methods Fifty two children with SSNS were en rolled in this study, randomized into observation group (27 cases) and control group (25). They were treated according to the standard for medium and long term steroid therapy plan generally applied domestically. Wutilinsi injection was given to those in oh servation group, oncea week for 12 weeks, urine protein was negative. The relapse during the treatment and within one year after that was investigated. A dynamic detection for adjuvanticity T-helper lymphocyte subsets was done in 15 cases in observation group and 13 in the control. Results The mean person time of relapse in observation group was lower (t=3.98,3.63;P〈0.01) and relapse-free cases were more than that in the control (X^2=27.66,P〈0.01). Before treated with steroid, the difference of the percentage of Thl and Th2 and the ratio of Thl/Th2 between the two groups was not significant. After administration of Wutilinsi, the percentage of Thl and the ratio of Th1/Th2 in observation group were significantly higher than those of the control group (t= 2.81,2.16;P〈0.05), though the percentage of Th2 was lower than that of control group, no difference was found between them (P)〉0.05). Conclusion Wutilinsi injection is effective for prevention of recurrence of SSNS. Its mechanism is related to adjusting Th1/Th2 function balance, accelerating the recovery of SSNS immune abnormality and improving the capability of anti infection.
