急性冠状动脉综合征患者IL-18和hs-CRP的检测意义

摘　　要：目的检测白细胞介素-18(IL-18)和超敏C反应蛋白(hs-CRP)在稳定型心绞痛(SAP)和急性冠状动脉综合征(ACS)患者中的水平,探讨其在ACS发病中的作用。方法对112例SAP患者、182例ACS患者和90例健康对照者的血清应用酶联免疫吸附试验法检测IL-18水平,应用增强免疫比浊法检测hs-CRP水平。结果ACS组与SAP组患者血清IL-18和hs-CRP水平显著高于对照组,ACS组的血清IL-18和hs-CRP水平又显著高于SAP组,SAP组和ACS组的IL-18和Hs-CRP呈显著正相关(P<0.01)。结论IL-18和hs-CRP与ACS的发病过程有关,可作为冠心病患者冠状动脉粥样硬化斑块稳定性的监测指标,对鉴别SAP和ACS有重要的参考价值。Objective To detect the levels of interleukin-18 （IL-18） and high sensitivity C reactive protein （hs- CRP） in 112 patients with stable angina pectoris （SAP） and 182 patients with acute coronary syndrome （ACS）, and to investigate its clinical significance in coronary artery disease. Methods 112 SAP patients （SAP group）, 182 ACS patients （ACS group） and 90 healthy controls （healthy control group） were enrolled in the study. Enzyme linked immunosorbent assay （ELISA） was applied to detecting the serum level of IL-18, and latex-enhanced immunoturbidimetry was employed to measure the serum level of hs-CRP. Results The serum levels of IL-18 and hs-CRP in SAP group and ACS group were significantly higher than those in healthy control group, especially in ACS group, the levels of IL-18 and hs-CRP was markedly higher than those in SAP group. There was a positive correlation between IL-18 and hs-CRP in SAP group and ACS group （P〈0.01）. Conclusion IL-18 and hs-CRP are associated with the occurrence and development of ACS, which may be used as monitoring indices of atherosclerotic plaque stability. They play an important role in the differential diagnosis of SAP and ACS.
