复方苦参注射液配合胸腔灌注化疗治疗肺癌胸腔积液的临床研究

摘　　要：目的:研究复方苦参注射液静脉滴注配合卡铂、胸腺五肽胸腔灌注化疗治疗肺癌胸腔积液的疗效。方法:选取48例肺癌恶性胸腔积液住院患者,随机分为复方苦参注射液静脉滴注配合卡铂、胸腺五肽胸腔灌注化疗组(观察组)和单纯卡铂胸腔灌注化疗组(对照组),从胸腔积液缓解情况、生活质量变化、细胞亚群变化、血浆sCD44sT的变化、毒副反应等方面进行临床疗效观察和评价。结果:观察组胸腔积液缓解疗效优于对照组(P<0.05);在改善肺癌胸腔积液患者生活质量(卡氏评分)方面,前者优于后者;对照组治疗后CD3+、CD4+、CD4+/CD8+明显低于观察组(P<0.05)而观察组血浆sCD44s明显低于对照组;观察组较对照组消化道反应轻、骨髓抑制轻,两组比较有显著性差异(P<0.05)。结论:苦参注射液静脉滴注配合卡铂、胸腺五肽胸腔灌注治疗肺癌胸腔积液疗效明显,毒副反应轻。Objective：Study the curative effects on the treating lung cancerous hydrothorax with compound radix sophorae flavescentis injection intravenous drip combining carboplatin and thymopentin intrapleural injection. Methods：48 sufferer being in hospital were selected with lung cancer malignant thorax hydrops, and were divided into compound radix sophorae flavescentis injection intravenous drip combining carboplatin and thymopentin intrapleural injection group（Test group） and simplely carboplatin intrapleural injection group （Check group）. Clinical curative effects were observed and estimated on hydrothorax lysis condition, living quality change, T cell subgroup change, the change of blood plasma sCD44s, and toxicity response. Results：Hydrothorax lysis curative effects in test group are better than in check group （P〈0.05）. In improving lung cancer hydrothorax sufferer＇s living quality, the former has precedence over the later. After treating, check group is obviously less than the test group in CD3^＋,CD4^＋,CD4^＋/CD8^＋（P〈0.05）. While the test group is obviously less than the check group in the blood plasma sCD44s. Comparing with the check group, the test group has the less in enteron response and marrow restraining. There is remarkable difference existing in the two groups （P〈0.05）. Conclusion：Curative effects on the treating lung cancerous hydrothorax with compound radix sophorae flavescentis injection intravenous drip combining carboplatin and thymopentin intrapleural injection is visible and less toxicity.
