Terumo导管经桡动脉冠状动脉造影对老年冠心病患者的诊断价值

摘　　要：目的探讨使用Terumo导管在老年冠心病患者经桡动脉行冠状动脉造影中的安全性、可行性及其诊断价值。方法共216例老年冠心病患者,包括稳定型心绞痛86例(39.8%)、不稳定型心绞痛72例(33.3%)、心肌梗死12例(5.5%)、缺血性心肌病24例(11.1%),其它22例(10.2%)。均先行Allen试验,桡动脉穿刺,5F Terumo导管分别行选择性左、右冠状动脉造影。结果208例成功进行了桡动脉造影,成功率为96.3%。造影结果发现,有意义的冠状动脉病变140例(67.3%),单支病变74例(52.9%),双支病变43例(30.7%),三支病变23例(16.4%)。68例属冠状动脉正常和非有意义的冠状动脉病变。8例经桡动脉造影失败者均改用股动脉Judkin法造影成功。结论采用Terumo导管桡动脉途径行冠状动脉造影在老年人冠心病患者安全可行,而且容易包扎止血、术后不须卧床,并发症和创伤均小,值得推广和应用。Objective To evaluate the feasibility, methology and diagnostic value of transradial angiography with Terumo catheters in the eldly. Methods There are 216 patients enrolled,with stable angina in 86 cases （39.8%）, unstable angina in 72 cases（33.3%）, myocardial infarction in 12cases（5.5%）, ischemic cardiomyopathy in 24 cases （11.1%） and others in 22 cases （10.2%）. Allen tests were carried in all patients firstly, then after local anesthesia, radial artery was punctured and a 5Fr introducer was inserted into the artery, and then selective coronary angiography was performed with Tertmo catheters. Results Transradial angiography with Terumo catheters were carried successfully in 208 patients （96.3%）. Their angiography results showed as： there were 140 cases of significant coronary artery disease, with single vessel disease in 74 cases （52.9 %）, double vessel disease in 43 cases（30.7%）and triple vessel disease in 23 cases（16.4%）. There were 68 cases with normal coronary artery or non-significant coronary artery diseases. There were 8 patients with unseccessful transradial angiography who were earfled by tranfemoral Judkins catheters successfully. Conclusion Transradial angiography with Terumo catheters in the eldly are safe and feasible with a low risk of local composition and a high successful rate.
