青藤碱对膝骨性关节炎实验兔关节液及血清中TNF-α水平的影响

摘　　要：目的观察向膝关节腔内注射青藤碱对膝骨性关节炎实验兔关节液及血清中肿瘤坏死因子（TNF-α）表达的影响。方法本研究共选取36只实验兔，采用后肢膝关节伸直位石膏管型固定法将其制作成膝骨性关节炎动物模型。将造模成功的实验兔随机分为青藤碱组、透明质酸组及模型组，另同时选取4只正常实验兔纳入空白对照组。于造模成功后向青藤碱组实验兔膝关节腔内注射青藤碱，透明质酸组及模型组实验兔关节腔内则分别注射等量透明质酸和生理盐水，每周注射1次，空白对照组不给予特殊处理。分别于药物注射前及注射5周后检测各组实验兔关节液及血清中TNF-α水平。结果青藤碱组及透明质酸组实验兔经相应药物注射后，其血清及关节液中TNF-α表达水平均明显下降，骨性关节炎症状也得到显著缓解；进一步分析后发现，青藤碱组TNF-α表达下降幅度明显大于透明质酸组（P〈0．05）。结论青藤碱能显著降低OA实验兔关节液及血清中TNF-α表达水平，其抗炎功效明显优于透明质酸。Objective To investigate the influence of an intra-articular injection of sinomenine （SN） on the expression of tumor necrosis factor-alpha （TNF-α） in synovial fluid and serum during osteoarthritis （ OA）. Method A rabbit model of degenerative osteoarthritis was created in the knee joint by applying a full extension plaster cast. The animals were randomly divided into a sinomenine group, a hyalurunic acid group, a model group and a blank control group. Six weeks after the model had been established, intra-articular injections of SN, byaluronic acid, or normal saline were administered once a week for 5 weeks to the members of the sinomenine, hyaluronic acid and model groups. The blank control group received no treatment. The levels of TNF-alpha in the synovial fluid of the knee and in the serum were measured before and after treatment. Result TNF-alpha in synovial fluid and serum was significantly decreased in the sinomenine and hyaluronic acid groups compared with the model group. A significant difference in TNF-alpha expression in the synovial fluid and serum was found between the sinomenine group and hyaluronic acid group. Conclusion Sinomenine can inhibit expression of TNF-alpha in synovial fluid and serum during the early stage of traumatic OA, and it was more effective than hyaluronic acid.
