摘  要:目的研究子宫内膜癌患者组织和外周血白细胞中hMLH1基因启动子是否存在甲基化,及hMLH1基因启动子甲基化与临床病理特征的关系,探讨组织中hMLH1基因启动子甲基化状态和外周血白细胞是否相关。方法选取2003年8月至2004年12月在天津医科大学总医院住院手术的子宫内膜癌新鲜组织48例及外周血50例,不典型增生子宫内膜及外周血6例,正常子宫内膜组织及外周血34例。采用甲基化敏感性核酸内切酶酶切PCR方法检验hMLH1启动子甲基化状态。结果正常子宫内膜与子宫内膜不典型增生组织中hMLH1基因启动子均无甲基化。子宫内膜癌组织hMLH1启动子甲基化率(35.4%)明显高于正常子宫内膜(P<0.01)。子宫内膜癌组织和外周血中hMLH1基因启动子甲基化率与手术—病理分期有关,早期显著高于晚期。外周血hMLH1基因启动子甲基化和组织中hMLH1启动子甲基化的关系密切。结论hMLH1基因启动子甲基化与子宫内膜癌的发生、发展有关。血液中hMLH1基因启动子甲基化检测可能成为筛查子宫内膜癌高危人群的有效方法之一。Objective To explore whether there is methylation of the hMLH1 promoter in tissue and peripheral blood of endometrial carcinoma, and associations between methylation of the hMLH1 promoter and clinicopathological characteristics of endometrial carcinoma, and to study the relationship between the status of methylation of the hMLH1 promoter in tissue and peripheral blood lymphocyte. Methods We studied 48 tumor tissues and 50 cases of peripheral blood of endometrial carcinoma, 6 atypical hyperplasia, and 34 healthy individuals in General Hospital of Tianjin Medical University from August 2003 to December 2004. Methylation of the hMLH1 promoter region was analyzed with methylation-sensitive restriction endonucleases PCR. Results There was no methylation of the hMLH1 promoter in both normal endometrium and atypical hyperplasia. Methylation of the hMLH1 promoter was found in 35.4% endometrial carcinoma, which was obviously higher than that in normal endometrium. The rate of methylation of the hMLH1 in tissue and peripheral blood of endometrial carcinoma was associated with FIGO stage, and it was significantly higher in early stage than that in late stage. Peripheral blood exhibited close affinity with tumor tissue in methylation of hMLH1 promoter. Conclusion Methylation of hMLH1 promoter is associated with tumorigenesis and development of endometrial cancer. Detection of methylation of hMLH1 promoter in peripheral blood may be a valid means for screening the high-risk group of endometrial carcinoma.
