摘  要:目的研究冠心病斑块稳定性与T细胞增殖反应的关系。方法冠心病患者81例,依据病情分为急性心肌梗死组(AMI组,20例)、不稳定性心绞痛组(UAP组,34例)和稳定性心绞痛组(SAP组,27例),另选非冠心病患者22例作为对照组。测定各组T细胞对植物血凝素(phytohemagglutinin,PHA)、氧化低密度脂蛋白(oxLDL)和低密度脂蛋白(LDL)的增殖反应,C反应蛋白(CRP)对AMI组和UAP组T细胞oxLDL增殖反应的影响。结果AMI组和UAP组T细胞对5μg/ml PHA的增殖反应高于SAP组和对照组(P<0.05),T细胞对5μg/mloxLDL及1μg/ml oxLDL的增殖反应高于SAP组和对照组(P<0.05),T细胞对10μg/ml oxLDL、5μg/mloxLDL、1μg/ml oxLDL的增殖反应均高于对同浓度LDL的增殖反应(P<0.01),10μg/ml CRP使AMI组和UAP组的T细胞对5μg/ml oxLDL的增殖反应升高(P<0.01)。结论T细胞介导的细胞免疫在导致斑块的不稳定中可能起着重要的作用,而CRP可能通过调节对特异性抗原oxLDL的细胞免疫发挥其促炎作用。Objective To investigate the relationship betweem plaque stability of coronary heart disease and proliferative response of T cells. Methods A total of 103 subjects were recruited in the study, including 20 acute myocardial infarction(AMI) ,34 unstable angina pectoris(UAP), 27 stable angina pectoris(SAP) and 22 control subjects. The proliferative response of T cells to phy- tohemagglutinin(PHA), oxidized low density lipoprotein (oxLDL) and low density lipoprotein (LDL) was measured by MTS/PMS colorimetric assay in all subjects. In addition, the effect of C-reactive protein(CRP) on the proliferative response of T cells to oxLDL in the AMI group and UAP group was also measured. Results The proliferative response of T cells to PHA 5 μg/ml was significantly higher in AMI group and UAP group than in SAP group and control group (P 〈 0.05). The proliferative response of T cells to oxLDL(at doses of 5 μg/ml and 1μg/ml) was significantly higher in AMI group and UAP group (P〈0.05) than in other 2 groups. The prolifertive response of T cells to oxLDL(at doses of 10 μg/ml,5μg/ml and 1μg/ml) was significantly higher than to LDL(at the same doses as oxLDL) in AMI group and UAP group (P〈0.01). CRP (10 μg/ml) increased the proliferative response of T cells to oxLDL(5 μg/ml) in AMI group and UAP group (P〈0.01). Conclusion Cellular immunity mediated by T cells may play an important role in the instability of plaque, and CRP may promote the inflammation of atherosclerosis through the effects on the specific immune response to oxLDL.
