摘  要:目的:探讨产后子痫性脑病的MRI表现.方法:回顾性分析产后24h内发病的14例子痫患者的临床和影像学资料,其中产后血压正常者4例,临床均以抽搐而行颅脑MRI检查,其中DWI及增强扫描各4例,SWI扫描2例.结果:MRI示病灶主要位于大脑皮层下白质,脑内其它区域亦可受累,呈基本对称分布.T1WI呈等或稍低信号,T2WI及FLAIR呈高信号.DWI扫描中3例均为高信号,1例病灶呈高、低两种信号.增强扫描除1例线状强化外,余均未见明显强化.SWI扫描中1例呈颅内多发点状出血,平扫并没显示.结论:MRI检查能够明确产后子痫患者脑部病变的部位及性质,同时有助于解释其影像表现的病理基础,从而进一步指导治疗及预防并发症,并纠正既往将其等同于高血压脑病的说法.AIM: To investigate the MRI findings of postpartum eclamptic encephalopathy. METHODS: The clinical and radiologic data of 14 patients with eclampsia in 24 h postpartum were analyzed retrospectively. Four cases were normotension. MR examinations were performed because of twitchy symptom. All cases underwent MR plain scaning, and 4 of them underwent diffusion weighted imaging (DWI) and contrast-enhanced scaning. Two un- derwent susceptibility weighted imaging (SWI). RESULTS: MRI showed basal bilaterally symmetrical distributions of lesions, mainly located in subcortical white matter of cerebral hemisphere. Lesions were iso-intensity or slightly hypo-intensity on T1WI and hyper-intensity on T2WI and FLAIR. Lesions were hyperintensity in 3 cases and hypo- intensity and hyper-intensity in lesions in 1 case. Multi-punctate hemorrhage was found in 1 case performed by SWI. Lesions did not display obvious enhancement excluding 1 by contrast-enhanced scan. CONCLUSION: MRI examinations can identify the location, quality and degree, meanwhile, analyze the pathological foundation of postpartum eclamptic encephalopathy, which conduces to guiding treatment and preventing complication, meanwhile, retrieving it from previous hypertensive encephalopathy version.
