摘  要:目的探讨小猪慢性胰腺炎(CP)MR表现。方法西双版纳小型猪正常7头,CP 15头,其中Ⅰ期7头,Ⅱ期4头,Ⅲ期4头。MR扫描:梯度回波T1W,快速恢复快速自旋回波T2W,MRCP。观察胰腺形态、信号及胰管改变。结果各组间胰腺萎缩无显著差异(p>0.05)。正常组主胰管均未见显示,Ⅰ期CP主胰管平均直径0.6mm,Ⅱ期2.5mm,Ⅲ期为4.5mm。正常组和Ⅰ期CP T1WI胰腺信号等于或高于肝实质,Ⅱ期CP组有3头(75%)信号降低,Ⅲ期CP组胰腺信号均降低(100%)。正常组和Ⅰ期CP组MR各项指标均无显著性差异(p>0.05)。Ⅱ、Ⅲ期CP与正常或Ⅰ期CP T1WI胰腺信号,主胰管直径及分支胰管的显示情况均显著性差异(p<0.05)。主胰管扩张程度与CP严重程度有相关性(r=0.885,p<0.05)。结论常规MR能发现阻塞性CP,并能一定程度予以分期。Objective To study the features of experimental chronic pancreatitis (CP) in piglets on non-enhanced MR imaging. Methods Twenty-two piglets were included in the study. They were 7 normal piglets, 7 piglets with Grade 1, 4 piglets with Grade 2 and 4 piglets with Grade 3 CP pathology. MR imaging was performed on 1.5T MR scanner using a synergy body phased-array coil before histopathology, including abdominal axial fat-suppressed fast spoiled gradient-echo T1-weighted (GRE T1 W) , axial fat-suppressed fastrecovery fast spin-echo T2-weighted (FRFSE T2W) and MR cholangiopanereatography (MRCP). The morphology, signal intensity and pancreatic duct of the pancreas on MR images were noted. Results In the 22 piglets, no significant differences could be seen for pancreatic atrophy on MR imaging among groups (p 〉 0.05 ). The main pancreatic duct in normal group could not be depicted on MR images. The diameters of the main pancreatic duct were 0.6mm, 2.5mm and 4.5mm for grade 1 , 3 and 3 CP, respectively. The signal intensity of the pancreas was higher than that of the liver on GRE T1-weighted images, and was comparable to that of the liver on FRFSE T2W images in normal piglets and grade Ⅰ CP. The pancreatic signal intensity was decreased in 75% of CP grade 2 and 100% of CP grade 3, respectively. There were no significant differences for above pancreatic patterns between normal piglets and pigs with Grade 1 CP. The dilatation of the pancreatic duct was related to the severity of CP ( r = 0. 885 ,p 〈 0.05 ). Conclusion Experimental chronic obstructive pancreatitis can be detected and be partially graded by non-enhanced MR imaging.
