射频消融治疗腰椎间盘突出症的临床应用

摘　　要：目的:探讨等离子刀装置对腰椎间盘突出症射频消融方法及临床疗效。材料与方法:92例腰椎间盘突出症患者,在C型臂X线机电视引导下穿刺椎问盘,置入等离子消融电极刀进入髓核,进行射频消融处理。术后随访5～12个月。结果:射频消融作用时间平均3.5min;按照Macnab评定标准进行疗效评定,92例患者,优72例,良9例,可8例,差3例;优良率为88.0%,有效率为96.7%;未发现任何并发症。结论:用等离子刀装置进行椎间盘髓核消融是治疗腰椎间盘突出症的安全而有效的新方法,认真选择适应症,利用适当剂量和积极的手术后处理可以达到满意的临床疗效。Purpose： To explore the method and the therapeutic effect of plasma radio - frequency ablation （PRFA） to the lumbar disc herniation. Materials and Methods： 92 cases with lumbar disc herniation was performed by the plasma radio - frequency ablation. Under the guidance of the ＂Carm＂ X - ray machine, punctured the herniated disc, lay down the electrode through puncture pin. After the treatment, the patients were followed- up for 5 - 12 months. Results： The average time of the PRFA are 3.5 minutes, The results were assessed according to the Macnab criteria. Of 92 patients, the effect was excellent in 72, good in 9, fair in 8 and poor in 3. The curative rate was 88%, the effective rate was 96.7%. No serious complication occurred in this group. Conclusion： PRFA is a very effective method to the lumbar disc herniation. The main factor to ensure the sueeessfien operation may depend on preoperative careful selection of the indication, proper dosage of the radio frequency ablation and the active treatment during the postoperation.
