摘 要:目的探讨口服碳酸氢钠对挤压综合征家兔肾脏的保护作用。方法24只健康家兔随机均分为4组,均固定于兔台,且C,D组挤压右后肢上部5h后,A,C组自由饮用清水,B,D组自由饮用碳酸氢钠溶液24h后,取血清和尿行生化检测,取肾组织光电镜及免疫组化形态学观察。结果C,D两组BUN,Cr较A组高,CO2CP及血清Mb较A组低(均P<0.05);C组K+较A组高及尿pH较A组低(均P<0.05),而D组无差异(均P>0.05);D组K+,BUN及Cr较C组低,而血清CO2CP,Mb及尿pH较C组高(均P<0.05)。A,B两组结构正常;C组肾小球内皮细胞重度肿胀,足细胞足突肿胀、完全融合,肾小管上皮细胞胞浆空化、部分细胞崩解脱落,管型成分均质物、红细胞、细胞碎片,D组肾小球内皮细胞肿胀,足细胞足突肿胀、轻度融合,肾小管上皮细胞胞核水肿、核旁空泡线粒体肿胀,管型成分均质物、红细胞。A,B两组肾脏未见肌红蛋白管型;C,D两组肾小管中有肌红蛋白管型,主要存在于远曲小管,但D组较C组轻。结论碳酸氢钠对挤压综合征家兔肾脏的保护作用。Objective To investigate the protection effect of oral administration sodium bicarbonate on rabbit ren from Crush syndrome. Methods 24 health rabbits were divided into 4 groups equally, fixed on platforms. Further right hind limb of rabbits were crushed 5h in Groups C and D. 24h after rabbits draught clear water in Groups A and B while sodium bicarbonate in Groups C and D. blood-serum , urine and nephridial tissue were observed. Results BUN,Cr of serum in Groups C and D were higher than these in Group A, while CO2CP(p〈0.05) ,Mb of serum were lower; K^+ of serum in Group C was higher while pH of urine was lower than that in Group A(p〈0. 05), there was no difference between Group A and D(p〈0.05), K^+ BUN,Cr were higher while CO2CP,Mb and pH of urine were lower in Group D than Group C. structures of Groups A and B were normal, glomerulus endothelioeyte heavy swell, foot process swell and coalesce completely, endoehylema of renal tubular epithelial cell cavitations, some cell disruption, Cast were made in homogeny, akaryoeyte, cell debris in Group C. glomerulus endothelioeyte swell, foot process swell and coalesce, endoehylema of renal tubular epithelial cell cavitations, some cell disruption, Cast were made in homogeny, akaryoeyte in Group D. myohemoglobin cast in Groups A and B were no found while these in Groups C and D were found, but these in Group C were less than that in Group D. Conclusions The effect of sodium bicarbonate on rabbit ren from Crush syndrome was utility.
