自制探针配合骨科丝锯治疗高位肛瘘40例疗效观察

摘　　要：为更准确寻找肛瘘内口，避免探针探查时造成假道，同时彻底清除瘘道管壁硬变及腐败组织，试验组采用自制探针配合骨科丝锯锯刮后，按瘘管走行于肛管直肠环上下的不同分别行切开挂线和挂线引流治疗高位肛瘘40例，并与对照组采用低切高挂术治疗40例高位肛瘘进行对照观察。两组间疼痛持续时间、创口愈合时间及瘢痕面积均有显著差异（P〈0．05）。结果表明，试验组各临床疗效指标明显优于对照组，并保证了术后肛门外观和括约肌功能正常。The objective of the study was to find the internal opening of anal fistula and avoid forming false canal of anal fistula when probing examination, thoroughness clean away hardened and necrotic tissues of anal fistula wall. Forty patients in the study group underwent scrape of the fistula with the self-made probe and orthopedix silk saw first, then underwent incision threading, threading and drainage according to the fistula alignment arround the anorectal ring,while another 40 cases in the controlled group underwent the procedure of low incision and high threading. Result showed that significant difference existed in pain-lasting time,wound healing time and cicatricial area in the two groups（ P〈0. 05）. It is revealed that the clinical therapeutic indexes in the study group were notably better than those in controlled group, including normal contour of the anus and normal function of the sphincter.
