血清P-选择素水平与老年2型糖尿病肾病的相关性

摘　　要：目的:探讨血清P-选择素水平与老年2型糖尿病肾病的相关性。方法:对65例老年2型糖尿病患者[其中正常白蛋白尿(DM-NA组)33例;微量白蛋白尿(DM-MA组)32例]和34例健康对照者(NC组)测定空腹血清P-选择素浓度,同时检测空腹血糖、胰岛素、C肽、糖化血红蛋白、血脂,以及血小板参数等指标;并分析血清P-选择素水平与指标间的相关性。结果:(1)DM-MA组和DM-NA组的血清P-选择素浓度均高于NC组(P<0.01),DM-MA组明显高于DM-NA组(P<0.001),差异有显著性。2型糖尿病患者的P-选择素与UAER、MPV呈中度正相关。结论:2型糖尿病患者的血清P-选择素水平高于正常人,血清P-选择素浓度的检测对判断糖尿病肾病的发生和发展有一定价值。Objective： To investigate the association of serum P-selectin levels with elderly type 2 diabetic nephropathy in Qingdao Han population. Methods-A case-control study on 65 senile patients with type 2 diabetes mellitus [including a normal alhuminuria group（gg individulas）and a microalhuminuria group （32 individulas）]and 34 normal control,serum P-selectin levels were measured by ELISA method. Fasting plasma glucose ,fasting serum insulin,fasting serum C peptide ,glycated haemoglobin （HbAlc）, serum lipids concentrations ,platelet parameters ,and so on,were also measured at the same time in all subjects. Finally to analysis the association of serum P-selectin levels with the observed indexes. Results：The serum P-selectin levels in type 2 diabetes mellitus group and its two subgroups were higher than the normal control group （P〈 0.01）. Serum P-selectin concentrations were also significantly increased in the microalhuminuria group compared with the normal albuminuria group. The concentrations of serum P-selectin in type 2 diabetes mellitus group had positive relationship to urinary microalbumin excretion rate and mean platelet volume. Conclusions： The serum P-selectin concentrations of the type 2 diabetes mellitus patients are higher than the normal individulas. The examination of the serum P-selectin concentrations may be of some value to judge the genesis and advancement of diabetic nephropathy.
