加味建瓴汤对高血压患者胰岛素抵抗及血清TNF-α水平的影响

摘　　要：目的:通过观察加味建瓴汤对阴虚阳亢并伴有痰湿型原发性高血压患者的胰岛素抵抗和血清TNF-α水平的影响,探讨其临床作用机制。方法:将符合纳入标准的原发性高血压患者60例,随机分为治疗组30例,对照组30例。对照组予依那普利治疗,治疗组予依那普利及加味建瓴汤治疗,疗程为3周,观察其治疗前与治疗后血压、胰岛素抵抗指数及血清TNF-α的水平。结果:(1)治疗组较对照组治疗前血压(SBP)、治疗后血压(DBP)、空腹血浆葡萄糖(FPG)、空腹胰岛素水平(FINS水平)有明显降低,具有显著性差异(P<0.01)。(2)治疗组较对照组患者ISI有明显改善,且差异显著(P<0.01)。(3)治疗组与对照组比较患者血清TNF-α水平明显下降,具有显著性差异(P<0.01)。结论:加味建瓴汤可明显改善阴虚阳亢伴痰湿型高血压患者胰岛素抵抗程度及血清TNF-α水平;加味建瓴汤疗效确切,是一个安全、有效的制剂。Objective:To study the curative effect mechanism of Jiaweijianling Decoction on insulin resistence and TNF-α level in patients of EH , which were Yin deficiency and Yang excess syndrome with phlegm-dampness. Method: A total of 60 EH patients were observed according to the diagnosis criteria. They were divided into two groups at random. The treatment group and the control group both had 30 patients. The control group were treated by west medicine, while patients in the treatment group were treated by Jiaweijianling Decotion everytime twice every day on the basis of west medicine. The course of treament lasted three weeks, then the changes of laboratory indexes、TNF-α level and the degree of insulin resistence were observed. Result: (1) In the treatment group ,the level of SBPBP、FPG、FINS were more remarkably reduced than in control group, and the difference was significant betreen the two groups (P<0. 01). (2) The level of ISI in the treatment group were improved more remarkably than in control group,and there was statistical difference between the two groups(P<0. 01). (3)The level of TNF-α in treatment group was decreased more significantly than in control group, and the difference between the two groups was statistical P<0. 01). Conclusion; Jiaweijianling Decoction could improve the level of SBP,DBP,FPG,FINS、TNF-α and the degree of insulin resistence of the EH patients. It is a safe and effective preparation and it is worthy to be studied deeply.
