摘 要:目的:研究替米沙坦与贝那普利对原发性高血压病患者血压和肾功能的影响。方法:80例1级、2级高血压病患者随机分为治疗组(40例,口服替米沙坦80mg/d,晨顿服)和对照组(40例,口服贝那普利10~20mg/d,1次/d)。测定2组患者用药前后血压的变化情况以及尿微蛋白排泄率、内生肌酐清除率、血和尿β2微球蛋白和N-乙酰-B-D氨基葡萄糖苷酶水平。结果:与治疗前相比,治疗12周后2组降压效果、尿微蛋白排泄率、血和尿β2微球蛋白和N-乙酰-B-D氨基葡萄糖苷酶均显著下降,内生肌酐清除率明显升高(P<0.05),组间比较差异无统计学意义(P>0.05);2组不良反应发生率分别为5%和13%,组间比较有显著差异(P<0.05)。结论:替米沙坦对高血压病1级、2级患者早期肾损害不仅有保护作用,而且不良反应轻、给药方便、依从性高。Aim:To study the influence of telmisartan and benazepril on renal function in patients with primary hypertension. Methods:A total of 80 primary hypertension patients were randomly divided into therapy group( n = 40, telmisartan,80 mg/d,qd) and control group ( n = 40, benazepril, 10 - 20 rag/d, qd). The blood pressure, blood and urinary β2 microglobulin (β2-MG) , urinary microalbumin excretion (UMAE) , reatinine clearance (Ccr) , and urinary NAG en- zyme(NAGE) were detected before and after 12 weeks treatment respectively. Results: After 12 weeks of treatment, blood pressure, UMAE, β2-MG and NAGE significantly decreased and Ccr had obvious rise in 2 groups( P 〈 0.05 ). There was no significant difference between 2 groups( P 〉 0.05 ). The adverse reaction rate was 5% for telmisartan group, and 13% for benazepril group . There was significant difference between 2 groups ( P 〈 0.05 ). Conclusion :Telmisartan can protect renal function in patients with primary hypertension class 1 and class 2 with less adverse reactions and higher compliance.
