硝普钠、多巴胺和多巴酚丁胺治疗扩张型心肌病顽固性心力衰竭疗效观察

摘　　要：目的:探讨硝普钠、多巴胺和多巴酚丁胺治疗扩张型心肌病顽固性心力衰竭的临床疗效。方法:32例患者给予休息、吸氧、利尿剂、地高辛、血管紧张素转换酶抑制剂(ACEI)类药物常规处理,存在心律失常,给予相应的抗心律失常治疗,同时给予5%葡萄糖注射液250ml+硝普钠10mg+多巴胺40mg+多巴酚丁胺40mg,以10滴/min静脉滴注,3～5d后症状缓解停用硝普钠,继续使用多巴胺和多巴酚丁胺,疗程15d。结果:32例患者治疗后心功能有显著改善(均P<0.05)。结论:硝普钠、多巴胺和多巴酚丁胺治疗扩张型心肌病顽固性心力衰竭疗效明显,安全可靠。Objective：To explore the clinical effect of sodium nitroprusside,dopamine with dobutamine in treatment of refractory heart failure of dilated cardiomyopathy. Methods：32 cases received regular treatment such as rest, oxygen, diuretic, digoxin, ACEI etc. When arrhythmia existed the drugs for anti-arrhythmia was given. Meanwhile sodium nitroprusside, dopamine with dobutamine were given too. After 3-5 days of symptom relieved, sodium nitroprusside was stoped, dopamine and dobutamine continued, the whole treatment course is 15 days. Results： The heart function in 32 cases improved obviously after treatment（P〈0.05）, Conclusion： The curative effect of sodium nitroprusside, dopamine with dobutamine in treatment of refractory heart failure of dilated cardiomyopathy is obvious, safety and credibility.
