中药熏洗对痉挛型脑瘫患儿肌张力及关节活动度改善情况的临床观察

摘　　要：目的:观察中药熏洗前后脑瘫患儿肢体肌张力和关节活动度的改善情况。方法:对30例痉挛型脑瘫患儿进行中药熏洗,分别于每次熏洗前、后对患儿进行肌张力评级,同时对髋关节被动屈曲角度、膝关节被动屈曲角度进行测量,评定中药熏洗前后患儿肌张力改善情况,以及关节活动度的改善情况,以评价中药熏洗的即刻疗效。结果:对熏洗后肌张力、髋关节、膝关节的改善情况进行即刻疗效分析,其总有效率分别达80%、83.3%、86.7%,说明患儿经熏洗后的即刻疗效较好;熏洗后肌张力评分明显较及熏洗前降低,P<0.01,有显著性差异,髋关节、膝关节被动屈曲角度(即刻屈曲角度)较熏洗前增加,P<0.05,有显著性差异,说明经熏洗后患儿肌张力以及髋关节、膝关节活动度改善情况明显优于熏洗前。结论:应用中药熏洗能够在短时间内降低痉挛型脑瘫患儿的肌张力,增加关节活动度,为推拿及功能训练打下良好基础,减轻患儿康复训练中的痛苦,充分体现中医药整体化治疗的优势。Objective. To observe the limb muscle tension and the range of joint improved situation of children with cerebral palsy before and after Chinese medicine fumigation. Methods ： Apply Chinese medicine fumigation to 30 cases of children with spastic cerebral palsy, evaluate the children muscle tension before and after Chinese medicine fumigation, and measure the hip and knee buckling angle at the same time, evaluate the immediate effect of traditional Chinese medicine fumigation. Results： After Chinese medicine fumigation analysis the immediate effect about improved situation of muscle tension, hip joint, knee joint, the total efficiency are 80%, 83.3%, 86.7%, P 〈 0.01, after Chinese medicine fumigation the instant effect is better than before;after Chinese medicine fumigation the muscle tension score is lower than before, P 〈0.01 ,there was a significant difference,the passive buckling angle （instantly buckling angle） of the hip joint, knee joint is bigger than before, P 〈 0.05, there was a significant difference, that is after Chinese medicine fumigation the muscle tension, hip joint range, knee joint range of the children is better than before it. Conclusion ： To use fumigation of Chinese medicine can be reduced the muscle tension in a short period of time in children with spastic cerebral palsy,increase rangement of the joint,lay a sound foundation for massage and functional training, reduce the pain of the rehabilitation training, and fully reflect the advantage of Chinese medicine in concept of wholism in treatment.
