摘  要:目的观察部分液体通气(PLV)对吸入性损伤犬呼吸力学、氧合和血流动力学参数的影响。方法16条健康杂种犬经蒸气吸入造成重度吸入性损伤模型,随机分为对照组和实验组(n=8)。实验组经气管导管缓慢注入氟碳液体(12ml/kg体重)行PLV治疗,治疗后30、60、90min时测定两组犬血气、气道阻力、肺顺应性及血流动力学参数。结果实验组PaO2呈进行性上升,在各时点与致伤后60min比较差异有统计学意义(P〈0.05)。与对照组比较,实验组各时点的PaO2稍有升高(P〉0.05)。两组气道阻力、肺顺应性和血流动力学参数比较,差异均无统计学意义(P〉0.05)。结论PLV有利于吸入性损伤犬的动脉氧合,对血流动力学无明显不利影响。Objective To investigate the influence of partial liquid ventilation (PLV) on the cardiopulmonary function in dogs with inhalation injury. Methods Sixteen mongrel dogs inflicted by hot steam inhalation were subjected to severe inhalation injury and were randomly divided into control and experiment groups. The dogs in experiment group were simultaneously supplied with perfluorocard liquid ( 12 ml/kg) into the lungs slowly via tracheal intubation for liquid ventilation. The blood gas analysis ,pulmonary compliance ,airway resistance and hemohynamic parameters were determined at 30,60 and 90 min after ventilation. Results The PaO2 in experiment group was increased progressively, which was significantly higher than the post-injury value at all time points ( P 〈 0.05 ). The PaO2 at all time points in experiment group was a little higher than that in control group ( P 〉 0.05 ). There was no difference between the two groups in terms of dynamic pulmonary compliance and airway resistance as well as the hemodynamics. Conclusion Partial liquid ventilation seemed to be beneficial to the oxygenation after inhalation injury and to be no influence on the hemodynamics.
