摘 要:目的探讨应用关节镜技术加微创锁定加压钢板方法治疗膝关节周围骨折的疗效。方法2006年8月至今采用关节镜监视下骨折复位加微创锁定加压钢板治疗膝关节周围骨折患者18例。男11例,女7例;年龄32~83岁,平均63.3岁。致伤原因:车祸5例,高处跌落4例,自行摔伤7例,其他2例,均为闭合性骨折。其中胫骨平台骨折16例,按Schatzker分型,Ⅰ型骨折7例,Ⅱ型骨折4例,Ⅲ型骨折5例,股骨远端骨折2例,均为股骨髁上骨折。18例患者均获随访,对其近期及远期并发症进行记录统计,对膝关节功能疗效按Lysholm评分表评估,得出优、良、中、差的例数及优良率。结果所有患者均无明显手术并发症,按Lysholm评分表评估膝关节功能,优15例、良2例、中1例,优良率94.4%。结论对于膝关节周围骨折患者,关节镜技术是一项有效的辅助治疗手段,加上微创技术与锁定加压钢板固定,膝关节周围骨折患者的治疗可以达到满意的疗效。Objective To investigate the application of treating fractures around the knee with arthroscope technology plus fixing compressed steel plate by micro-incision. Methods From August 2006,treating fractures around the knee by means of the fracture replace under the monitoring of arthroscope technology plus fixing compressed steel plate by micro-incision in 18 cases. 11 cases of male and 7 female patients;aged 32 to 83 years old with an average age of 63.3. Injury reasons:five cases by traffic accident,four cases by the drop from height, seven cases by losing their balance,two cases by other reason. All are closed type fractures. Tibial platform fracture of 16 cases,the type accord ing to Schatzker:seven cases of fracture type Ⅰ ,four cases of fracture type Ⅱ ,five cases of fracture type Ⅱ ,two cases of distal femur fractures ,all are condylar fracture. 18 patients have follow-up survey with carrying on the record and statistics of their short and long term complications. Evaluating the treatment effect to the knee's function according to Lysholm chart and obtaining the cases of excellent,good,medium,bad and the rate of fineness. Results All the patients had no significant postoperative complications. Evaluating the knee's function in short and long-term according to Lysholm chart:excellent in 15 case,good in 2 cases,medium in one case,fine rate is 94.4%. Conclusion For the patients with fractures around the knee,arthroscopic technique is an effective auxiliary treatment method,plusing micro-wound technology and fixing compressed steel plate,patients' fractures around the knee can be treated with satisfactory results.
