经尿道前列腺扩裂术与电化学治疗前列腺增生致急性尿潴留的疗效比较

摘　　要：目的比较经尿道前列腺扩裂术(TUPS)与经尿道前列腺电化学疗法(TUPE)治疗良性前列腺增生(BPH)致急性尿潴留(AUR)的疗效。方法随机采用TUPS与TUPE两种手术方式治疗BPH致AUR患者240例,其中TUPS组180例,TUPE组60例。对两组患者的国际前列腺症状评分(IPSS)、生活质量评分(QOL)、手术时间、留置尿管时间、最大尿流率(Qmax)、剩余尿量(RUV)、手术治愈率等进行比较。结果两组手术前后IPSS、QOL、Qmax、RUV比较均有改善,但两组之间改善程度差异有统计学意义(P<0.001)。TUPS组手术时间平均(9.4±2.1)min,TUPE组平均(146.5±29.3)min,TUPS手术操作时间显著短于TUPE(P<0.001)。TUPS组拔尿管早于TUPE组(P<0.01)。TUPS治愈率显著高于TUPE(P<0.001)。结论TUPS治疗BPH致AUR近期疗效满意,优于TUPE。Objective To compare the effects of the transurethral prostatic splitting operation（TUPS） and transurethral prostatic electrochemical therapy（TUPE） for treatment of acute urinary retention in benign prostatic hyperplasia（BPH） patients. Methods Two hundred and fourty cases of BPH were randomly treated with TUPS and TUPE. All patients were followed up with International Prostatic Symptom Score（IPSS）, quality of life （QOL）, operation time, indwelling catheterization, mean Qmax, residual urinary volume（RUV） and operation cure rates. Results IPSS, QOL, Qmax and RUV were significantly improved after operation in two treated groups,moreover level of changes in symptoms improvement were also obvious difference between these two treated group, （P〈0.001）. Mean operation time was（9.4±2.1） min inTUPS group and （146.5±29.3）min in TUPE group, the operation time was longer TUPE group than that in TUPS group（P〈0.001）, the catheterization time were less in TUPS group than that in TUPE group（P〈0.01）. The cure rates were higher in TUPS group than that in TUPE group（P〈0.001）. Conclusion The efficacy of TUPS for the treatment of BPH with AUR was more satisfactory and superior than that of TUPE.
