维持性血透患者对促红细胞生成素疗效与应用左旋卡尼汀关系的探讨

摘　　要：目的探讨应用左旋卡尼汀能否提高促红细胞生成素对慢性肾衰竭血透患者的疗效。方法将40例维持性血液透析的贫血患者随机分为两组，A组20例，透析后静脉注射rhEPO 3000 IU，每周2～3次。B组20例，透析后静脉注射左旋卡尼汀1g，同时应用rhEPO 3000 IU静脉注射每周2～3次治疗。观察两组病人治疗前及治疗后4周、8周、12周时的血红蛋白、红细胞压积、血浆白蛋白的变化。结果随着治疗的进行，A组、B组Hb、Hot显著升高，治疗后12周时与治疗前比较，差异有显著性（P〈0．05）；治疗后12周时A组与B组比较血红蛋白、红细胞压积、血浆白蛋白差异均有显著性（P〈0．05）。结论促红细胞生成素可改善维持性血液透析患者的贫血状态，联合应用左旋卡尼汀能提高rhEPO的疗效。Aim To investigate whether L-carnitine can further improve the effect of rhEPO in hemodialysis patients with chronic renal failure. Methods 40 patients with nephritic anemia were randomly divided into two groups, group A：20 patients rhEPO30OOIU intrave- nously infused after every dialysis 2 -3 times every week;group B：20 paatients accepted 3000 IU of rhEPO and L-earnitine 1.0 through vain injection 2 -3 times every week. Their serum albumin （ Alb）, hemoglobin（Hb） and hematoerit（ Het ）were monitored. Results Along with the treatment,Hb and Het in both groups inereased significantly. The values of them after 12 weeks＇ treatment were significantly higher than those before treatment. After 12 weeks＇ treatment,the hemoglobin and hematocrit and albumin values in group B were signifieantly higher than those in group A. Conclusion Erythropoietin can improve the anemia situation of the maintenance hemodialysis patients,and L-carnitine can enhance the curative effect of rhEPO.
