膝骨性关节炎患者关节液透明质酸含量变化及关节功能状态的观察

摘　　要：目的：观察不同年龄段膝骨性关节炎（osteoarthritis OA）患者关节液中透明质酸（Hyaluronic AcidF HA）变化，并分析膝OA患者的关节功能状态。方法：本研究包含二项病例对照研究。在一项临床研究中，60例膝骨性关节炎患者分别按年龄段分为30～40岁、40～50岁和大于50岁年龄段组，以10名平均年龄为38．7岁的正常人群作对照组，应用放射免疫测定法检测各组关节滑液样本中透明质酸的含量。在另一项病例对照研究中，病例组为33名膝OA病人，对照组为31名正常人。采用肌肉-骨骼-关节评估系统（Muscle Evaluation Svstem MES）进行包括下肢肌力、抗骨折能力、膝关节活动空间、静态平衡性、动态平衡性等项目的关节功能整体评估。结果：骨性关节炎患者滑液中透明质酸水平显著高于正常对照组（P〈0．05），但在各年龄段之间并无明显差别。正常组与骨性关节炎组在抗骨折能力、关节活动空间、静态平衡性及动态平衡性上无显著差异，但骨性关节炎组下肢肌力显著低于正常组（P〈0．05）。结论：膝骨性关节炎患者关节滑液中透明质酸水平明显升高，下肢肌力则明显降低。Objective:To observe the changes of hyaluronic acid(HA)in synovial fluid of patients in different stages with osteoarthritis(OA)in knee joint and analyze joint functional status of them.Methods:Two case control studies were included.In one research,60 cases of patients with OA in knee joint were divided into three groups according to the stages of their years,group A(20 cases,in the stage of 30 years to 40 years),group B(20 cases,between 40 years and 50 years)and group C(20 cases,exceeding 50 years),group D(control group,10 cases of health adults with averagely 38.7 years).The contents of HA in samples of joint synovial fluid were detected with radioimmunoassay in four groups.In the other,33 cases of patients with OA in knee joint were selected as case group,and 31 cases of health adult were for control group,they were totally evaluated their joint functions(including animal force of lower limbs,capability of anti-fracture,space of knee joint motion,stat-ic equilibrium and dynamic equilibrium)with Muscle Evaluation System(MES).Result:The levels of HA in synovial fluid of patients with OA were significantly super to those of control group(P<0.05),there had no significant difference on different stages.On capability of anti-frac-ture,space of knee joint motion,static equilibrium and dynamic equilibrium,there had no significant deviation between control group and OA group,but on animal force of lower limbs,OA group was notably infra to control group(P<0.05).Conclusion:For patients with OA in knee joint,the levels of HA in joint synovial fluid obviously raised,on the contrary animal force of lower limbs degraded.
