儿童福氏志贺氏菌OXA型β内酰胺酶耐药基因及其耐药性检测

摘　　要：目的了解儿童志贺氏菌是否存在OXA基因及其发生率以及对耐药性的影响。方法对2006年6-11月从本院细菌性腹泻患儿粪便培养标本分离到的福氏志贺氏菌共51株,进行β内酰胺酶OXA基因PCR检测,对PCR产物进行DNA序列分析并与标准株进行比对,确定其亚群并用K-B法测定志贺氏菌对7种抗生素的敏感度。结果51株福氏志贺氏菌中45株检出OXA-1群基因,阳性率为88.23%;福氏志贺氏菌对氨苄西林的耐药率较高,OXA-1群基因阳性志贺氏菌的耐药率为91.11%、OXA-1群基因阴性志贺氏菌的耐药率为83.33%;对氨苄西林-舒巴坦也有一定的耐药,OXA-1群基因阳性的为26.67%、OXA-1群基因阴性的为33.33%;对头孢曲松的耐药率为OXA-1群基因阳性的26.67%、OXA-1阴性的为0。51株福氏志贺氏菌中未检出OXA-10群基因。结论从福氏志贺氏菌检出OXA-1型β内酰胺酶耐药基因,携带率为88.23%。该基因与福氏志贺氏菌对β内酰胺类抗生素的耐药率无相关性。Objective To investigate the OXA-type β-lactamase gene in Shigella flexneri and its prevalence in children, as well as the impact of the gene on bacterial resistance. Methods Fifty-one strains of S. flexneri were isolated from faecal specimens of children with bacterial diarrhea between June 2006 and November 2006 at Children＇s Hospital of Fudan University. OXA-type β-lactamase gene was detected by polymerase chain reaction （ PCR ）. The susceptibility of S. flexneri strains to 7 antimicrobial agents was determined by K-B method. Results OXA-1 gene was detected in 45 （ 88. 23% ） strains of S. flexneri. The S. flexneri isolates were highly resistant to ampicillin. The resistance rate of OXA-lgene positive S. flexneri was 91.11% ,and that of OXA-1 gene negative S. flexneri was 83.33%. About 26. 67% of OXA-1 gene positive S. flexneri and 33.33% of OXA-1 gene negative S. flexneri were resistant to ampicillin-sulbactam. About 26. 67% of OXA-1 gene positive S. flexneri was resistant to ceftriaxone and none of OXA-1 gene negative S. flexneri was resistant to eeftriaxone. OXA-10 gene was not detected in 51 strains of S. flexneri. Conclusion About 88.23% of S. flexneri can be detected OXA-1 gene. The OXA-1 gene has no relationship with the resistance rate of β-lactam antibiotics in the S. flexneri isolates.
