摘  要:目的探讨血浆脑钠肽(type B natriuretic peptide,BNP)水平与舒张性心力衰竭并同时存在肾功能不全,心房纤颤间的关系。方法通过对356例心衰患者临床症状,心脏彩色多普勒超声检查和有创血流动力学监测的结果,筛选出确诊的肯定舒张功能不全患者67例。评价肯定的舒张功能不全患者BNP浓度,以及肾功能不全,心房纤颤对BNP浓度的影响。结果肯定的舒张功能不全患者血浆BNP浓度升高,但显著低于收缩功能不全患者,合并心房纤颤,肾功能不全的患者血浆BNP浓度明显升高。结论舒张功能不全的患者血浆BNP浓度也会增加,但程度不如收缩功能不全患者。合并心房纤颤患者血浆BNP浓度会增高,肾功能不全会影响BNP的排泄,血浆BNP浓度也会增高。Objective To investigate the relationship between brain natriuretic peptides(BNP) and diastolic heart failure.To investigate whether renal dysfunction and atria fibrillation could influence BNP level.Methods We diagnosed 67 patients with clinical heart failure syndromes as positive diastolic heart failure by the results of color doppler echocardiography and swan-ganz's flowing balloon catheter. Results Patients who were diagnosed as positive diastolic heart failure have higher BNP level than normal person,but lower than patients with systolic heart failure. Patients with renal dysfunction and atria fibrillation also have higher BNP level. Conclusion BNP level of patients with positive diastolic heart failure would increased, Renal dysfunction and atria fibrillation can also influence the level of BNP.
