摘 要:目的探讨MR扩散加权成像(DWI)在女性盆腔病变诊断中的价值。方法对135例女性盆腔病变进行常规MR平扫和DWI,表面扩散系数(b值)分别为0s/mm2和1000s/mm2,测量病变的表面扩散系数(ADC)值,其中16例同时行增强扫描。结果43例子宫恶性病变的平均ADC值为(0.90±0.10)×10-3mm2/s,24例子宫良性病变的平均ADC值为(0.12±0.09)×10-3mm2/s,2组病变之间比较有统计学意义(P<0.01);34例附件区恶性病变的平均ADC值为(0.98±0.11)×10-3mm2/s,34例附件区良性病变的平均ADC值为(2.25±0.12)×10-3mm2/s,2组病变之间比较有统计学意义(P<0.01)。结论DWI有助于对女性盆腔良恶性病变的诊断和鉴别。Objective To investigate the value of diffusion - weighted imaging( DWI ) in diagnosing the feminine pelvic lesions. Methods Conventional MRI and DWI with b value of 0 and 1000 s/mm^2 respectively were performed in 135 feminine patients with pelvic lesions. Apparent diffusion coefficient (ADC) values of the lesions were measured. Contrast- enhancement MR imaging was performed in 16 patients. Results The average ADC values were (0.90±0.10) × 10^-3mm^2/s in uterine malignant lesions (43 cases) , (0. 12 ± 0.09) × 10^-3mm^2/s in uterine benign lesions ( 24 cases ) , ( 0.98 ±0.11 ) ×10^-3mm^2/sin adnexal malignant diseases ( 34 cases ) and ( 2.25 ±0.12) × 10^-3mm^2/s in adnexal benign ones( 34 cases), respectively. There were significant differences between malignant and benign groups(P 〈 0.01). Conclusion DWI is a useful method to differentiate benign from malignant pelvic lesions.
