摘  要:目的:评价增菌荧光定量PCR(FQ-PCR)检测痰及血液中结核杆菌的临床应用价值。方法:对61例菌阴和难治性结核患者的痰液、血液以及增菌痰液、增菌血液分别进行FQ-PCR检测,比较相互之间的差异性。结果:痰液、增菌痰液FQ-PCR检测阳性率分别为27.87%(17/61)、40.43%(19/47),二者比较2χ=3.92,P<0.05,差异有统计学意义。血液、增菌血液FQ-PCR检测阳性率分别为0(0/35)、22.5%(9/40),二者比较2χ=8.94,P<0.005,差异有统计学意义。痰液总阳性率为45.90%(28/61),较血液总阳性率22.5%(9/40)高,二者比较2χ=5.70,P<0.05,差异有统计学意义。结论:增菌FQ-PCR在菌阴性和难治性结核杆菌的检测中可作为结核病的辅助诊断之一。痰液FQ-PCR阳性率明显高于血液,但痰液、血液联合FQ-PCR检测可提高结核杆菌的检出率。Objective:To determine the clinical value bacteria for detection of MTB FQ-PCR in sputum and blood.Methods:From 61 patients of negative TB sputum,blood or sputum by bacteria proliferation,conducted by FQ-PCR,and compare the differences between each other.Results:The positive rates of FQ-PCR before and after bacteria proliferation were 27.87%(17/61),40.43%(19/47) compared the two group χ2=3.92,P〈0.05,the difference is statistically significant.Blood and blood of bacteria proliferation FQ-PCR positive rates were 0(0/35),22.5%(9/40),χ2=8.94,P〈0.005,the difference statistically significant.The total positive rate of sputum is 45.90 percent(28/61),and blood is 22.5%(9/40),χ2=5.70,P〈0.05,the difference is statistically significant.Conclusions:Bacteria proliferation accompanied FQ-PCR for detection of Mycobacteria tuberculosis can be used as the supplementary diagnosis of negative bacteria and refractory tuberculosis Sputum FQ-PCR positive rate was significantly higher than that of blood,sputum,blood joint FQ-PCR detection met had can increase the detection rate of TB.
